摘  要:目的:探讨舍曲林联合脑电生物反馈方法对首发抑郁症的治疗作用及对血清细胞因子水平的影响。方法:将140例首发抑郁症患者分为联合组和对照组各70例;均给予入组者盐酸舍曲林治疗,联合组患者辅助脑电生物反馈治疗;疗程均为8周;治疗前后采用汉密尔顿抑郁量表(HAMD)评估入组者临床症状;治疗前后检测血清同型半胱氨酸(Hcy)、星形胶质源性蛋白(S100β)、5-羟色胺(5-HT)、超敏C反应蛋白(hs-CRP)、白细胞介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)水平;观察治疗过程中两组不良反应情况。结果:治疗后,两组HAMD评分明显低于治疗前,且联合组显著低于对照组(P均<0.01);联合组临床疗效明显优于对照组(Z=12.559,P<0.01);治疗后,两组血清Hcy、S100β水平较治疗前显著降低,且联合组显著低于对照组;两组血清5-HT水平较治疗前显著升高,且联合组显著高于对照组(P均<0.05);两组血清TNF-α、IL-2、hs-CRP水平明显低于治疗前,且联合组显著低于对照组(P均<0.05);不良反应发生率两组比较差异无统计学意义(P>0.05)。结论:联合脑电生物反馈方法可明显增加舍曲林治疗抑郁症的疗效,其作用机制可能与调节细胞因子水平有关。Objective:To investigate the therapeutic effect of sertraline combined with EEG biofeedback on first-episode depression and its effect on serum cytokine levels.Method:140 patients with first-episode depression were divided into the combination group and the control group,70 cases in each group;the patients in the two groups were treated with sertraline hydrochloride,and on this basis,the patients in the combination group were assisted with EEG biofeedback therapy.for 8 weeks.Before and after treatment,the Hamilton Depression Scale(HAMD) was used to evaluate the clinical symptoms of the participants.Serum homocysteine(Hcy),astrocyte-derived protein(S100β),5-hydroxytryptamine(5-HT),high-sensitivity C-reactive protein(hs-CRP),interleukin-2( IL-2),tumor necrosis factor-α(TNF-α) levels were tested before and after the treatment.The adverse reactions of the two groups during the treatment were observed.Results:After treatment,the HAMD scores of the two groups were significantly lower than those before treatment,and the combined group was significantly lower than the control group(P0.01).Conclusion:The combined EEG biofeedback method can significantly increase the efficacy of sertraline in the treatment of depression,and its mechanism of action may be related to the regulation of cytokine levels.
