维A酸片联合蒽环类药物治疗急性早幼粒细胞白血病的疗效及对血脂及凝血功能的影响

摘　　要：目的探究维A酸片联合蒽环类药物治疗急性早幼粒细胞白血病(APL)的效果及其对血脂及凝血功能的影响。方法200例APL患者为研究对象,随机分为观察组与对照组,每组100例。对照组采用蒽环类药物治疗,观察组在对照组基础上采用维A酸片治疗。对比两组治疗相关指标、治疗效果、凝血功能指标[活化部分凝血活酶时间(APTT)、纤维蛋白原(FBG)]及血脂指标[低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)]。结果观察组凝血指标恢复正常时间(6.17±1.41)d、血液学指标缓解时间(22.17±1.11)d均短于对照组的(11.21±2.51)、(29.21±1.77)d,差异有统计学意义(P<0.05)。观察组治疗总有效率93.00%高于对照组的60.00%,差异有统计学意义(P<0.05)。治疗后,观察组APTT(34.29±1.91)s短于对照组的(37.36±1.81)s、FBG(2.69±0.91)g/L高于对照组的(2.21±0.82)g/L,差异均有统计学意义(P<0.05)。观察组TC、TG、LDL-C水平分别为(4.21±1.20)、(1.42±0.32)、(2.41±1.21)mmol/L,均低于对照组的(5.83±1.31)、(2.23±1.54)、(2.98±1.21)mmol/L,HDL-C(1.46±0.21)mmol/L高于对照组的(1.01±0.22)mmol/L,差异均有统计学意义(P<0.05)。结论维A酸片联合蒽环类药物治疗APL效果显著,能够改善患者凝血功能以及血脂指标,值得研究和推广。Objective To investigate the effect of tretinoin tablets combined with anthracyclines in the treatment of acute promyelocytic leukemia(APL)and its influence on blood lipids and coagulation function.Methods A total of 200 APL patients were studied and randomly divided into observation group and control group,with 100 cases in each group.The control group was treated with anthracyclines,and the observation group was treated with tretinoin tablets on the basis of the control group.Both groups were compared in terms of treatment-related indexes,therapeutic effect,coagulation indexes[activated partial thromboplastin time(APTT),fibrinogen(FBG)]and lipid indexes[low-density lipoprotein cholesterol(LDL-C),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)].Results The recovery time of coagulation indexes(6.17±1.41)d and remission time of hematological indexes(22.17±1.11)d in the observation group were shorter than(11.21±2.51)and(29.21±1.77)d in the control group,and the differences were statistically significant(P<0.05).The total effective rate of treatment 93.00%in the observation group was higher than 60.00%in the control group,and the difference was statistically significant(P<0.05).After treatment,the APTT(34.29±1.91)s in the observation group was shorter than(37.36±1.81)s in the control group,and the FBG(2.69±0.91)g/L was higher than(2.21±0.82)g/L in the control group,and the differences were statistically significant(P<0.05).The TC,TG,and LDL-C levels in the observation group were(4.21±1.20),(1.42±0.32),and(2.41±1.21)mmol/L,which were lower than(5.83±1.31),(2.23±1.54),and(2.98±1.21)mmol/L in the control group;the HDL-C(1.46±0.21)mmol/L in the observation group was higher than(1.01±0.22)mmol/L in the control group;the differences were all statistically significant(P<0.05).Conclusion Tretinoin tablets combined with anthracyclines in the treatment of APL have a significant effect,and can improve the coagulation function and blood lipid indexes of patients,which is worthy
