摘  要:目的:探讨飞秒激光制瓣的准分子激光原位角膜磨镶术(FS-LASIK)与有晶状体眼后房型人工晶状体(ICL)植入术治疗高度近视的效果及对术后高阶像差的影响。方法:选取160例(320眼)高度近视患者为研究对象,根据手术方式不同分为ICL植入术组和FS-LASIK组,每组各80例(160眼)。比较两组患者术后6个月裸眼视力(UCVA)、等效球镜度、角膜高阶像差和对比敏感度。结果:术后6个月,两组患者UCVA均较术前增高(P<0.05),等效球镜度均较术前降低(P<0.05),但组间比较无统计学差异(P>0.05);两组患者角膜高阶像差各项指标均增高(P<0.05),且FS-LASIK组高于ICL植入术组(P<0.05);ICL植入术组6 c/d、9 c/d空间频率下的对比敏感度较术前增高(P<0.05),且高于FS-LASIK组(P<0.05)。结论:FS-LASIK与ICL植入术矫治高度近视,均能够有效矫正屈光,提高裸眼视力,但ICL植入术后高阶像差增加较小,视觉质量较好。Objective:To investigate the effect of femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and posterior chamber intraocular lens(ICL)implantation in the treatment of high myopia and its influence on postoperative high order aberrations.Methods:160 patients(320 eyes)with high myopia were selected as the research objects.The patients were divided into ICL implantation group and FS-LASIK group according to different surgical methods,with 80 cases(160 eyes)in each group.The differences in postoperative UCVA,equivalent spherical degree,high-order corneal aberration and contrast sensitivity at 6 months after operation between the two groups were compared.Results:At 6 months after operation,the UCVA increased compared with before operation in both groups(P<0.05),the spherical equivalent decreased compared with before operation(P<0.05),the difference between groups was not statistically significant(P>0.05).At 6 months after operation,the corneal high-order aberration indexes in the two groups were significantly increased(P<0.05).Compared with the FS-LASIK group,the corneal high-order aberration indexes in the ICL implantation group were decreased(P<0.05).At 6 months after operation,the contrast sensitivity of ICL implantation group at 6 c/d and 9 c/d spatial frequency was significantly higher than that before operation(P<0.05),and was higher than that of FS-LASIK group(P<0.05).Conclusion:FS-LASIK and ICL implantation in the treatment of high myopia can effectively correct refraction and improve naked eye vision.The increase of high-order aberrations after ICL implantation is small,and the visual quality is good.
