以“补脾强肾”为主的特定穴推拿对脑性瘫痪患儿脑血流状态的影响

摘　　要：目的探讨“补脾强肾”原则指导下的特定穴推拿对脑性瘫痪(简称脑瘫)患儿运动功能、综合康复效果及脑血流状态的影响。方法选取2020年7月至2022年2月我院康复中心脑瘫患儿102例作为研究对象,随机分为对照组和观察组各51例。对照组给予常规推拿,观察组在对照组基础上加用“补脾强肾”指导下小儿推拿疗法中的特定穴推拿,对比两组患儿临床综合康复疗效、粗大运动功能、精细运动功能及脑血流速度。结果干预后,观察组总有效率为94.12%(48/51),高于对照组80.39%(41/51),差异有统计学意义(P<0.05);观察组粗大运动功能、精细运动功能评分高于对照组,差异有统计学意义(P<0.05);两组患儿大脑前动脉、大脑中动脉、大脑后动脉血流速度均较干预前升高,观察组高于对照组,血管搏动指数较干预前降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论在传统推拿手法的基础上加强以“补脾强肾”为主的特定穴推拿,具有提高脑血流灌注水平等优势,能有效提高运动发育水平,帮助脑瘫患儿综合功能康复,效果确切。Objective To explore the effect of massage at specific acupoints under the principle of"invigorating the spleen and strengthening the kidney"on the motor function,comprehensive rehabilitation effect and cerebral blood flow in children with cerebral palsy.Methods Select 102 children with cerebral palsy treated in our department from July 2020 to February 2022 as the research subjects,and divide them into two groups(51 in each)by random number table method.The control group was given routine massage,and the observation group was additionally given massage at specific acupoints in pediatric massage therapy under the principle of"invigorating the spleen and strengthening the kidney".The clinical comprehensive rehabilitation effect,gross motor function,fine motor function and cerebral blood flow velocity of the two groups were compared.Results After intervention,the total effective rate of the observation group was 94.12%,higher than that of the control group,which was 80.39%(P<0.05);the gross motor function and fine motor function scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05);the blood flow velocities of the anterior cerebral artery(ACA),middle cerebral artery(MCA),and posterior cerebral artery(PCA)in the two groups were all higher than those before the intervention,and the observation group was higher than the control group,and the vascular pulsatility index was lower than before intervention,and it was lower in the observation group than the control group(P<0.05).Conclusion On the basis of traditional massage techniques,strengthening the specific acupoint massage mainly focusing on"invigorating the spleen and strengthening the kidney"has the advantages of improving the level of cerebral blood perfusion,which can effectively improve the level of motor development,and help children with cerebral palsy to recover comprehensively with good effect.
