摘 要:目的研究化学萃取同种异体肌腱重建跟腱缺损的效果。方法选取成年新西兰大白兔45只,体质量2.5~3.0 kg。取其中15只兔的跟腱,采用化学萃取法进行抗原灭活制备同种异体肌腱,取剩余30只兔制备跟腱缺损模型并行异体肌腱移植,将同种异体肌腱移植至缺损跟腱后,随机分为三组(每组10只)。术后6周、12周、24周每组各取兔的移植后肌腱并抽血,大体观察后采用HE染色观察细胞核数量,评估三组同种异体肌腱组织内细胞生长情况,CD 4+T、CD 8+T观察评估肌腱免疫排斥,拉力实验测试肌腱拉伸情况。结果经HE染色,采用化学萃取法制备的同种异体肌腱抗原被灭活且纤维组织结构保持完好,术后6周HE染色示,B组肌腱组织内的细胞核密度明显多于A组,C组多于B组,差异有统计学意义(P<0.05)。各时间点CD 4+T、CD 8+T随着时间增加,免疫兼容性随之提高。术后6周Masson染色示,B组肌腱组织中的细胞核明显增多,C组肌纤维及胶原纤维相互交错,组织结构更加紧凑,肌腱组织以蓝染为主,而A组细胞核较少,主要为原移植物的胶原纤维。力学拉伸实验也表明,化学萃取后肌腱随时间增加,力学性能增强。结论经化学萃取法灭活抗原的同种异体肌腱能够治疗兔跟腱缺损,且随随访时间延长,跟腱拉力较前明显增强,且与自体组织兼容性随之提高。Objective To study the effect of chemically extracted allogeneic tendon for reconstruction of Achilles tendon defect. Methods Forty-five adult New Zealand white rabbits with a body weight of 2.5~3.0 kg were used the Achilles tendons of 15 rabbits were taken and antigen was inactivated by chemical extraction method to prepare allogeneic tendon. The remaining 30 rabbits were taken to prepare Achilles tendon defect model and allogeneic tendon transplantation was carried out. After allogeneic tendon transplantation to the defect Achilles tendon, the rabbits were randomly divided into 3 groups(10 rabbits in each group).At 6, 12 and 24 weeks postoperatively, the transplanted tendons of each group were collected and blood was drawn. After general observation, HE staining was used to observe the number of nuclei,and the cell growth in the tendon tissue of three groups was evaluated. The immune rejection of the tendon was evaluated by CD 4+T and CD 8+T observation. The tension test measured the tension of the tendon. Results After HE staining, the allogenic tendon antigen prepared by chemical extraction method was inactivated and the fibrous tissue structure remained intact. Six weeks after surgery, HE staining showed that the nucleus density of the tendon tissue in group B was significantly higher than that in group A, and that in group C was higher than that in group B, with statistical significance(P<0.05). CD 4+T and CD 8+T at each time point showed better immune compatibility with increasing time. Masson staining 6 weeks after surgery showed that the nuclei of tendon tissue in group B were significantly increased, while muscle fibers and collagen fibers interlaced with each other in group C, resulting in A more compact tissue structure. The tendon tissue was mainly blue staining, while the nuclei in group A were less, mainly collagen fibers of the original graft.
