血清IL-6、IL-8及IL-32水平对多发性骨髓瘤患者肾损伤的诊断价值

摘　　要：目的探讨多发性骨髓瘤(MM)患者中血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及白细胞介素-32(IL-32)的表达及其预测肾损伤的价值。方法选取2017年1月至2020年5月在本院收治的74例MM患者作为MM组。依据国际分期系统(ISS)分为Ⅰ期(14例)、Ⅱ期(25例)和Ⅲ期(35例)。根据肾功能情况,将其分为肾损伤组(26例)和无肾损伤组(48例)。另选取同期50例健康体检正常者作为对照组。检测各组血清IL-6、IL-8、IL-32及β2-微球蛋白(β2-MG)水平。应用ROC曲线分析血清IL-6、IL-8、IL-32及β2-MG水平诊断MM患者肾损伤的价值。结果MM组的血清IL-6、IL-8、IL-32及β2-MG水平均高于对照组,差异均有统计学意义(均P<0.001)。随着ISS分期的增加,血清IL-6、IL-8、IL-32及β2-MG水平增加,差异均有统计学意义(均P<0.001)。肾损伤组的血清IL-6、IL-8、IL-32及β2-MG水平均高于无肾损伤组(均P<0.001)。ROC曲线分析结果显示IL-6、IL-8、IL-32及β2-MG联合诊断MM患者发生肾损伤的曲线下面积(AUC)最大(AUC=0.958,95%CI:0.902~0.993),其灵敏度和特异度为97.2%和80.4%。结论MM患者的血清IL-6、IL-8、IL-32水平明显升高,且与MM患者的病情分期及肾损伤有关,其联合β2-MG检测对MM患者肾损伤具有良好诊断价值。Objective To investigate the expression of serum interleukin-6(IL-6),interleukin-8((IL-8)and Interleukin-32(IL-32)in patients with multiple myeloma(MM)and their value in predicting renal injury.Methods The seventy-four MM patients admitted to our hospital from January 2017 to May 2020 were divided into phase I(14 cases),phase II(25 cases)and phase II(35 cases)according to the international staging system.According to renal function,MM patients were divided into renal injury group(26 cases)and non-renal injury group(48 cases).Another 50 normal persons in the same period were selected as the control group.The levels of serum IL-6,IL-8,IL-32 andβ2-microglobulin(β2-MG)were detected in each group.Receiver operating characteristic(ROC)curve were used to analyze the value of serum IL-6,IL-8,IL-32 andβ2-MG levels in the diagnosis of renal injury in MM patients.Results The serum levels of IL-6,IL-8,IL-32 andβ2-MG in the MM group were higher than those in the control group,and the differences were statistically significant(all P<0.001).The serum levels of IL-6,IL-8,IL-32 andβ2-MG increased with the increase of ISS stage,and the difference was statistically significant(P<0.001).The serum levels of IL-6,IL-8,IL-32 andβ2-MG in the renal injury group were higher than those in the non-renal injury group(P<0.001).ROC curve analysis showed that the combination of IL-6,IL-8,IL-32 andβ2-MG had the largest area under the curve(AUC)for the diagnosis of renal injury in MM patients(AUC=0.958,95%Cl:0.902-0.993),and its sensitivity and specificity were 97.2%and 80.4%.Conclusions The serum levels of IL-6,IL-8 and IL-32 in MM patients are significantly increased,which are related to the disease stage and renal injury of MM patients.Combined withβ2-MG detection has a good diagnostic value for renal injury in MM patients.
