基层医院肿瘤科560例营养不良患者治疗的回顾性分析

摘　　要：目的了解基层医院肿瘤科患者营养不良的发生率及治疗现状。方法收集本院肿瘤科2021年3月至9月经营养风险筛查≥3分诊断为营养不良住院1周以上的患者,分为常规营养治疗组、肠内营养治疗组、肠外营养治疗组、肠内肠外肠内肠外联合治疗组,比较出院与入院时患者的体质量指数、血红蛋白、淋巴细胞、白蛋白、前白蛋白的差值,差值为正是治疗有效,差值为0或负是治疗无效,统计各组各项指标的有效率并进行卡方检验。结果本院营养不良发生率为42.6%,560例患者纳入统计,营养治疗率70.5%。肠内营养治疗组的各项指标有效率均为最高,在体质量指数、白蛋白、前白蛋白的有效率上,差异有统计学意义(P<0.05)。结论基层医院肿瘤患者营养不良发生率高,肠内营养治疗效果佳,营养规范化治疗需要进一步加强。Objective To investigate the incidence and treatment status of malnutrition in oncology patients in primary hospitals.Methods The patients who were diagnosed with malnutrition and hospitalized for more than 1 week in the oncology department of our hospital from March to September 2021 with nutritional risk screening score≥3 were collected and divided into no nutrition therapy group,enteral nutrition therapy group,parenteral nutrition therapy group,and enteral+parenteral therapy group.The body mass index,hemoglobin,lymphocytes,albumin,and prealbumin were compared between discharge and admission.If the difference was positive,the treatment was effective,and if the difference was 0 or negative,the treatment was ineffective.The effective rate of each index in each group was counted and tested by chi-square.Results The incidence of malnutrition in our hospital was 42.6%.560 patients were included in the statistics.The nutritional treatment rate was 70.5%.The effective rates of each index in the enteral nutrition treatment group were the highest,with P<0.05 in the effective rates of body mass index,albumin and prealbumin.There was no statistical significance in the effective rates of hemoglobin and lymphocytes significance.Conclusion The incidence of malnutrition in cancer patients in primary hospitals is high,and enteral nutrition therapy has the best effect.The standardized nutrition therapy still needs to be further strengthened.
