摘  要:目的 研究酒精性肝病患者CYP2E1基因多态性与氧化应激的关系及临床意义。方法 将2019年9月至2022年1月期间,在内蒙古科技大学包头医学院第二附属医院就诊的60例酒精性肝病患者纳入研究组,将同期体检、有长期饮酒史且肝功能正常的80例患者纳入对照组。检测外周血CYP2E1基因多态性(C1/C1型、C1/C2型、C2/C2型)及血清中肝功能指标[白蛋白(Alb)、丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、γ-谷氨酰转肽酶(γ-GT)]、氧化应激指标[活性氧簇(ROS)、丙二醛(MDA)、8-羟基脱氧鸟嘌呤(8-OHdG)、过氧化脂质(LPO)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH)、总抗氧化力(T-AOC)]。结果 研究组的C1/C1基因型比例、C1等位基因频率均高于对照组,C1/C2基因型比例、C2等位基因频率均低于对照组(P<0.05);研究组中C1/C1型患者的血清ALT、AST、γ-GT、ROS、MDA、8-OHdG、LPO含量高于C1/C2型、C2/C2型患者,SOD、GSH、T-AOC含量低于C1/C2型、C2/C2型患者。结论 CYP2E1基因C1/C1基因型与酒精性肝病发病及肝功能损害加重、氧化应激反应激活有关。Objective To study the relationship between CYP2 E1 gene polymorphism and oxidative stress in patients with alcoholic liver disease and its clinical significance.Methods A tatol of 60 patients with alcoholic liver disease treated in The second affiliated hospital of Baotou medical college from September 2019 to January 2022 were included in the study group,and 80 patients with normal physical examination,normal liver function and long-term drinking history were selected as the control group.CYP2 E1 gene polymorphism in peripheral blood(C1/C1,C1/C2,C2/C2)and the contents serum albumin(Alb),alanine aminotransferase(ALT),aspartate aminotransferase(AST),γ-glutamyl transpeptidase(γ-GT),reactive oxygen species(ROS),malondialdehyde(MDA),8-hydroxydeoxyguanine(8-OHdG),lipid peroxide(LPO),superoxide dismutase(SOD),glutathione peroxidase(GSH)and total antioxidant capacity(T-AOC)were detected.Result The ratio of C1/C1 genotype and C1 allele frequency in the study group were higher than those in the control group,and the ratio of C1/C2 and C2/C2 genotype and the frequencies of C2 allele were lower than those in the control group(P<0.05).The content of ALT,AST,γ-GT,ROS,MDA,8-OHdG and LPO of C1/C1 patients in study group were higher than those of C1/C2 and C2/C2 patients,and the contents of SOD,GSH and T-AOC were lower than those of C1/C2 and C2/C2 patients(P<0.05).Conclusion The C1/C1 genotype of CYP2 E1 gene is related to the pathogenesis of alcoholic liver disease,the aggravation of liver function damage and the activation of oxidative stress response.
