彩超对肝硬化门脉高压症部分脾栓塞术前后脾静脉、门静脉的监测价值

摘　　要：目的探究彩超对肝硬化门脉高压症部分脾栓塞术前后脾静脉、门静脉的监测价值。方法收集2018年12月-2020年10月于本院行部分脾栓塞术治疗的肝硬化门脉高压症患者10例。使用彩色多普勒超声监测10例患者术前、术后1周、术后3个月脾静脉与门静脉的血管内径、血流速度、血流量。结果术后1周、术后3个月脾静脉及门静脉平均血流速度、血流量均较术前降低(P<0.05);术后1周、术后3个月脾静脉内径均较术前显著缩小(P<0.05);脾静脉平均血流速度术后3个月较术后1周有所增加。术后患者脾厚及脾上下端径与术前比较,差异无统计学意义(P>0.05)。结论彩色多普勒超声能有效监测肝硬化门脉高压症部分脾栓塞术前后脾静脉、门静脉的血管内径、血流速度及血流量,利于对脾动脉栓塞效果的监测,具有显著临床价值。Objective To explore monitoring value of color Doppler ultrasonography on splenic vein and portal vein before and after partial splenic embolization of patients with liver cirrhosis and portal hypertension.Methods The paper chose 10 patients with liver cirrhosis and portal hypertension with partial splenic embolization in our hospital from December 2018 to October 2020,blood vessel diameter,blood flow velocity and blood flow of splenic vein and portal vein of 10 patients before operation,1 week after operation and 3 months after operation was monitored with Color Doppler ultrasound.Results Mean blood flow velocity and blood flow of splenic vein and portal vein decreased 1 week and 3 months after operation compared with those before operation(P<0.05);internal diameter of splenic vein decreased significantly 1 week and 3 months after operation compared with that before operation(P<0.05);mean blood flow velocity of splenic vein increased 3 months after operation compared with 1 week after operation.There was no significant difference in splenic thickness and diameter of upper and lower ends of spleen between patients after and before operation(P>0.05).Conclusion Color Doppler ultrasound can monitor diameter,blood flow velocity and blood flow of splenic vein and portal vein effectively before and after partial splenic embolization of patients with cirrhosis and portal hypertension,which is helpful to monitor effect of splenic artery embolization with significant clinical value.
