摘  要:目的探讨外周血G蛋白耦联雌激素受体30(GPR30)表达水平在脑小血管病(CSVD)致认知功能障碍患者中的变化情况,并分析其与认知功能障碍发生风险、病情程度及预后的关系。方法将2018年7月—2020年12月收治的CSVD致认知功能障碍92例作为观察组,CSVD后无认知功能障碍92例作为对照组,比较2组外周血单个核细胞(PBMC)中GPR30蛋白与mRNA表达情况,分析PBMC中GPR30蛋白、mRNA与认知功能障碍发生风险、病情程度的关系,并评价二者对预后的影响及预测价值。结果观察组PBMC中GPR30蛋白相对灰度值、mRNA相对表达量低于对照组(P<0.01)。重度认知功能障碍患者PBMC中GPR30蛋白相对灰度值、mRNA相对表达量低于中度、轻度患者,中度患者低于轻度患者(P<0.05)。PBMC中GPR30蛋白相对灰度值、mRNA相对表达量与认知功能障碍发生风险、病情程度呈负相关(P<0.05)。预后不良患者年龄、美国国立卫生研究院神经功能缺损量表评分、认知功能障碍程度高于预后良好患者,PBMC中GPR30蛋白相对灰度值、mRNA相对表达量低于预后良好患者(P<0.01)。在校正其他因素前后,PBMC中GPR30蛋白相对灰度值、mRNA相对表达量均与预后独立相关(P<0.01)。PBMC中GPR30蛋白相对灰度值及GPR30 mRNA相对表达量均对CSVD致认知功能障碍预后有较好的预测价值。结论外周血GPR30在CSVD致认知功能障碍中呈显著下调趋势,与认知功能障碍发生风险、病情程度显著相关,对预后预测价值较高。Objective To investigate the changes in the expression level of G protein-coupled estrogen receptor 30(GPR30)in peripheral blood of patients with cognitive dysfunction caused by CSVD,and to analyze its relationship with the risk,severity and prognosis of cognitive dysfunction.Methods A total of 92 patients with cognitive dysfunction caused by CSVD in our hospital from July 2018 to December 2020 were selected as the observation group,and 92 patients without cognitive dysfunction after CSVD were selected as the control group.The expression of GPR30 protein and mRNA in peripheral blood mononuclear cells(PBMC)of the two groups were compared.The relationship between GPR30 protein and mRNA in PBMC and the risk and severity of cognitive dysfunction was analyzed,and their impact on and predictive value in prognosis were evaluated.Results The relative gray value and mRNA expression level of GPR30 protein in PBMC in observation group were lower than those in control group(P<0.01).The relative gray value of GPR30 protein and the relative mRNA expression level in PBMC of patients with severe cognitive dysfunction were lower than those in moderate and mild patients,and lower in moderate patients than in mild patients(P<0.05).The relative gray value of GPR30 protein and the relative mRNA expression level in PBMC were negatively correlated with the risk and severity of cognitive dysfunction(P<0.05).The age,National Institute of Health stroke scale(NIHSS)score and degree of cognitive dysfunction in patients with poor prognosis were higher than those in patients with good prognosis,while the relative gray value of GPR30 protein and relative mRNA expression level in PBMC were lower than those in patients with good prognosis(P<0.01).Before and after adjustment for other factors,the relative gray value of GPR30 protein and relative mRNA expression in PBMC were independently correlated with prognosis(P<0.01).The relative gray value of GPR30 protein and the relative expression level of GPR30 mRNA in PBMC showed good predictive value f
