双期增强CT对腮腺基底细胞腺瘤和多形性腺瘤的诊断价值

摘　　要：目的探讨双期增强CT对腮腺基底细胞腺瘤(BCA)和多形性腺瘤(PA)的诊断价值.方法回顾性分析经手术病理确诊的腮腺BCA患者40例以及PA患者100例的影像资料,对两者进行对比分析,以病理诊断结果为金标准,采用受试者工作特征(ROC)曲线进行双期增强CT值阈值、敏感性、特异性的计算.结果BCA组患者年龄显著大于PA组,PA组患者年龄跨度显著大于BCA组,差异有统计学意义(t=7.178,9.116,P<0.05);BCA组患者病灶大小显著小于PA组(t=6.427,P<0.05);动脉期以及静脉期增强幅度比较,BCA组均显著大于PA组(t=6.185,5.173,P<0.05);ROC曲线分析显示动脉期曲线下面积(AUC)为0.904,静脉期为0.893;以动脉期60HU为阈值,其诊断的敏感性为89.58%,特异性为84.37%.结论腮腺BCA及PA双期增强CT表现各不相同,动脉期CT值>60HU为BCA,<60HU为PA.ObjectiveTo explore the diagnostic value of dual-phase enhanced CT in patients with basal cell adenoma(BCA)and pleomorphic adenoma(PA)of parotid gland.MethodsThe imaging data of 40 patients with BCA and 100 patients with PA confirmed by surgery and pathology were analyzed retrospectively.According to the pathologically gold standard,receiver operating characteristic(ROC)curve was performed to calculate the threshold,sensitivity and specificity of dual-phase enhanced CT.Results The age of patients with BCA group was significantly greater than that of patients with PA group,while the age span of patients with PA group was significantly greater than that of patients with BCA group(t=7.178,9.116,P<0.05).The lesion size of patients with BCA group was significantly smaller than that of patients with PA group(t=6.427,P<0.05).The arterial and venous phase enhancement amplitudes of patients with BCA group were significantly greater than those of patients with PA group(t=6.185,5.173,P<0.05).The ROC curve analysis showed that the area under the curve(AUC)of the arterial phase was 0.904,and the venous phase was 0.893,respectively.Based on the arterial phase 60 HU as the threshold,the diagnostic sensitivity was 89.58%,and the specificity was 84.37%,respectively.Conclusion The imaging features of BCA and PA of parotid gland are differently presented in dual-phase enhanced CT,showing greater than 60 HU for BCA and less than 60 HU for PA in arterial phase.
