糖皮质激素结合前庭功能训练对前庭神经炎患者平衡功能、炎症因子及氧化应激水平的影响

摘　　要：【目的】探讨糖皮质激素联合前庭功能训练治疗对前庭神经炎患者平衡功能、炎性因子及氧化应激水平的影响。【方法】将两院确诊并接受治疗的70例前庭神经炎患者随机分为观察组(n=35)和对照组(n=35),对照组采用前庭功能训练进行治疗,观察组在对照组的治疗基础上加用泼尼松片治疗,两周为1疗程,连续治疗2个疗程。比较两组患者治疗前后Berg平衡量表(BBS)评分、炎性因子[白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)]水平及氧化应激标志物[丙二醛(MDA)和超氧化物歧化酶(SOD)]水平及临床疗效。【结果】治疗后,两组患者BBS评分及血清SOD表达水平均显著高于治疗前(P<0.05),且观察组高于对照组(P<0.05)。治疗后,两组血清IL-6、TNF-α及MDA水平均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,观察组总有效率明显高于对照组(P<0.05)。【结论】糖皮质激素联合前庭功能训练治疗前庭神经炎,可提高患者的平衡功能,降低患者炎症因子水平,改善患者氧化应激水平,提高临床疗效。【Objective】To explore the effects of glucocorticoid combined with vestibular function training on the balance function,levels of inflammatory factors and oxidative stress in patients with vestibular neuritis.【Methods】A total of 70 patients with vestibular neuritis who were diagnosed and treated in our hospitals were divided into the observation group(n=35 cases)and the control group(n=35 cases).The patients in the control group received vestibular function training only,while patients in the observation group were treated with prednisone tablets in addition to vestibular function training.The treatment contained 2 consecutive courses of 2-week treatment per course.The score of Berg Balance Scale(BBS),the levels of inflammatory factors interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),the levels of malondialdehyde(MDA)and superoxide dismutase(SOD),the markers of oxidative stress before and after treatment,and the clinical effects were observed and compared between the two groups before and after treatment.【Results】After treatment,the BBS score and the level of serum SOD in the two groups were higher than before treatment(P<0.05).And those parameters of patients in the observation group were significantly higher than those of patients in the control group(P<0.05).Levels of serum TNF-α,IL-6 and MDA in the two groups after treatment were lower than those before treatment(P<0.05).Patients in the observation group showed significantly lower levels of TNF-α,IL-6 and MDA compared to patients in the control group(P<0.05).After treatment,the total effective rate of the observation group was higher than that the control group(P<0.05).【Conclusion】Glucocorticoid combined with vestibular function training can improve the balance function of patients with vestibular neuritis,reduce the level of inflammatory factors,improve the level of oxidative stress,and improve the clinical efficacy.
