摘  要:目的 分析湿热交换器在喉癌气切患者气道湿化中的应用价值。方法 选取厦门大学附属第一医院2019年7月-2021年8月收治的76例喉癌带气囊气切导管患者作为研究对象,根据患者的入院治疗时间分组,将2019年7月-2020年7月收治的喉癌患者纳入参照组(n=38),将2021年8月-2022年8月收治的喉癌患者纳入研究组(n=38)。参照组实施常规气道湿化干预,研究组采用湿热交换器进行气道湿化干预。比较两组患者痰液黏稠度、痰痂形成率、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PCO_(2))。结果 研究组与参照组的痰液黏稠度分布存在显著差异(Z=2.647,P<0.05)。研究组痰液黏稠度Ⅰ度占比18.42%,高于参照组的42.11%,痰液黏稠度Ⅲ度占比为34.21%,低于参照组的13.16%,痰液黏稠度Ⅱ度占比较为接近;研究组痰痂形成为39.47%,低于参照组的15.79%,差异有统计学意义(χ^(2)=5.330,P<0.05);两组患者的PaCO_(2)比较,差异无统计学意义(P>0.05);研究组PaO_(2)水平(93.68±4.74)mmHg,高于参照组的(86.32±4.23)mmHg,差异有统计学意义(t=7.142,P<0.05)。结论 采用湿热交换器可有效改善患者的血气分析指标,降低痰液黏稠度,减少痰痂形成,气道湿化效果显著,从而提升了患者的舒适度,也可降低并发症。Objective To analyze the application value of moist heat exchanger in airway humidification of laryngeal cancer patients with tracheostomy.Methods A total of 76 laryngeal cancer patients with balloon tracheostomy catheters who were admitted to the First Affiliated Hospital of Xiamen University from July 2019 to August2021 were selected as the research subjects,and the patients were grouped according to the time of admission and treatment.The laryngeal cancer patients admitted from July 2019 to July 2020 were included in the reference group (38 cases),and the laryngeal cancer patients admitted from August 2021 to August 2022 were included in the research group (38 cases).The reference group was given routine airway humidification intervention,while the research group was given airway humidification intervention with a humid heat exchanger.The sputum viscosity,sputum crust formation rate,arterial partial pressure of oxygen (PaO_(2)) and arterial partial pressure of carbon dioxide (PCO_(2)) were observed and compared between the two groups.Results There was a significant difference in the distribution of sputum viscosity between the study group and the reference group (Z=2.647,P<0.05).The proportion of sputum viscosity grade I was 18.42%higher than that of the reference group (42.11%),the proportion of sputum viscosity gradeⅢwas34.21%lower than that of the reference group (13.16%),and the proportion of sputum viscosity gradeⅡwas relatively close.The scab formation rate was 39.47%lower than that of the reference group (15.79%),and the difference was significant (χ^(2)=5.330,P<0.05);there was no significant difference in PaCO_(2) between the two groups (P>0.05);The PaO_(2) level of the study group (93.68±4.74)mmHg was higher than that of the reference group (86.32±4.23)mmHg,and the difference was statistically significant (t=7.142,P<0.05).Conclusion The use of a moist heat exchanger can effectively improve the blood gas analysis indicators of patients,reduce the viscosity of sputum,reduce the formation of sputu
