根管显微镜结合锥形束CT在复杂根管诊疗中应用及效果分析

摘　　要：目的:分析根管显微镜结合锥形束CT在复杂根管诊疗中的应用效果。方法:选择2019年2月至2021年4月需行复杂根管治疗的96个根管,采用锥形束CT检查以获取病变根管实际形态及根管钙化位置、范围,并在根管显微镜引导下使用超声波器械对钙化根管进行疏通,记录锥形束CT、根管显微镜在探查根管数目方面的差异,计算不同牙位、钙化类型根管疏通成功率。结果:各类探查方式相比,锥形束CT较完整、清晰地显示了96个根管病变根管形态、钙化位置以及变化趋势,检出率为100.00%,高于显微镜的88.54%(P<0.05)。其中92个根管得以成功疏通,疏通成功率为95.83%。根管中段至根尖1/3段钙化根管中4个根管因钙化物顶端至根尖段已完全钙化,无法顺利疏通,疏通成功率较非根管中段至根尖1/3段钙化疏通成功率低(P<0.05)。结论:于复杂根管诊疗中应用根管显微镜结合锥形束CT进行处理,可获得较为理想的根管疏通效果。Objective:To analyze the application effect of root canal microscopy combined with cone beam CT in the diagnosis and treatment of complex root canals.Method:Selection in February 2019-April 2021 requires complex 96 root canal,root canal treatment using cone beam CT examination to get the actual form and root canal calcification lesions in root canal location,range,and guided by using ultrasound equipment in the root canal microscope to unclog the calcified root canal,record the cone beam CT detection root canal number,root canal microscope differences,The success rate of root canal dredging was calculated for different tooth positions and calcified types.Results:Compared with all kinds of methods,cone beam CT showed 96 root canal lesions in a more complete and clear manner,the location of calcification and the trend of change,the detection rate was 100.00%,higher than that of microscope 88.54%(P<0.05).Ninety-two canals were successfully dredged,with a success rate of 95.83%.Calcification in the middle part of the root canal to 1/3 of the root tip,4 root canals could not be dredged successfully because the calcification from the tip to the root tip had been completely calcified,and the success rate of dredging was lower than that of non-calcification from the middle part to 1/3 of the root tip(P<0.05).Conclusion:In complex root canal diagnosis and treatment,the application of root canal microscope combined with cone beam CT can obtain a more ideal effect of root canal dredging.
