摘  要:目的 探索胸部恶性肿瘤(食管癌和肺癌)患者外周血白介素-6(interleukin-6,IL-6)、白介素-10(interleukin-10,IL-10)及转化生长因子(transforming growth factor-β1,TGF-β1)水平与放射性肺损伤(radiation induced lung injury, RILI)的关系。方法 回顾性分析武汉科技大学附属孝感医院2019年12月至2021年5月收治的60例食管癌(n=21)及肺癌患者(n=39),根据治疗后是否出现RILI分为RILI组(n=13)和非RILI组(n=47),用酶联免疫吸附测定法检测两组治疗前后的IL-6、IL-10、TGF-β1水平,并进一步分析该指标与RILI的相关性。结果 放疗结束时IL-6水平在RILI组及非RILI组间存在差异,且RILI组的IL-6水平升高(P<0.05);放疗结束时及放疗结束1月时IL-10水平在RILI组及非RILI组间存在差异,且RILI组的IL-10水平升高(均P<0.05);放疗结束时及放疗结束1月时TGF-β1水平在RILI组及非RILI组间存在差异,且RILI组的TGF-β1水平升高(P=0.003,P=0.037)。不同时期外周血IL-6、IL-10水平存在差异(均P<0.05)。放疗结束时IL-6倍增值在RILI组及非RILI组间存在差异,且RILI组的倍增值升高(P<0.001);放疗结束时IL-10倍增值在RILI组及非RILI组间存在差异,且RILI组的倍增值升高(P=0.016);放疗结束TGF-β1水平在RILI组及非RILI组间存在差异,且RILI组的倍增值升高(P=0.025)。放疗结束时IL-6倍增值与IL-10倍增值无相关性(r=0.248,P=0.056),与TGF-β1倍增值存在正相关(r=0.355,P=0.005,),IL-10倍增值与TGF-β1倍增值无相关性(r=0.234,P=0.071)。结论 胸部恶性肿瘤患者放疗结束时的IL-6、IL-10、TGF-β1倍增值与RILI存在正相关,且预测RILI发生的准确度较高,可成为潜在的预测RILI发生的预测因子。Objective To explore the relationship between the peripheral blood interleukin-6, interleukin-10, and transforming growth factor-β1 and radiation-induced lung injury in patients with thoracic malignant tumors(esophageal cancer and lung cancer). Methods A total of 60 patients with esophageal cancer(n=21) and lung cancer(n=39) admitted to Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from December 2019 to May 2021 were retrospectively analyzed. According to the presence of radiation-induced lung injury(RILI) after treatment, the patients were divided into the RILI group(n=13) and the non-RILI group(n=47). The levels of IL-6, IL-10, and TGF-β1 in the two groups before and after treatment were detected by enzyme-linked immunosorbent assay(ELISA), and the correlation between these indexes and RILI was further analyzed. Results At the end of radiotherapy, the level of IL-6 level was different between the RILI group and the non-RILI group, and the level of IL-6 was increased in the RILI group(P<0.05). IL-10 levels at the end of radiotherapy and 1 month after radiotherapy were different between the RILI group and non-RILI group, and IL-10 levels were increased in the RILI group(all P<0.05). The TGF-β1 level at the end of radiotherapy and 1 month after radiotherapy was different between the RILI group and the non-RILI group, and the level of TGF-β1 was increased in the RILI group(P=0.003,P = 0. 037). The levels of IL-6 and IL-10 in the peripheral blood of patients were different in different periods( all P< 0. 05). At the end of radiotherapy,the IL-6 fold increase was different between the RILI group and the non-RILI group and increased in the RILI group( P < 0. 001);the Il-10-fold increase at the end of radiotherapy was different between RILI group and non-RILI group,and the increase in RILI group was increased( P = 0. 016);The level of TGF-β1 at the end of radiotherapy was different between the RILI group and the non-RILI group,and the fold increase was increased in the RILI group( P = 0
