亮丙瑞林与米非司酮治疗子宫肌瘤的临床效果比较

摘　　要：目的比较亮丙瑞林和米非司酮治疗子宫肌瘤的临床效果。方法选择子宫肌瘤患者120例,按照随机数字表法分为观察组与对照组,每组60例。对照组给予米非司酮治疗,观察组给予注射用醋酸亮丙瑞林微球治疗。比较两组血管生长因子(VEGF)水平和性激素水平,子宫体积、子宫肌瘤体积及血红蛋白水平,总有效率及不良反应发生率。结果治疗前,两组VEGF水平及性激素水平比较,差异无统计学意义(P>0.05);治疗后,观察组VEGF水平低于对照组,促黄体生成素(LH)、雌二醇(E_(2))、卵泡刺激素(FSH)水平低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血红蛋白水平、子宫体积及子宫肌瘤体积比较,差异无统计学意义(P>0.05);治疗后,观察组血红蛋白高于对照组,子宫体积、子宫肌瘤体积小于对照组,差异有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论亮丙瑞林治疗子宫肌瘤可有效调节子宫肌瘤患者的VEGF及性激素水平,其缩小子宫肌瘤及体积作用显著,有助于提高患者的血红蛋白水平,治疗效果优于米非司酮,且安全性良好。Objective To compare the clinical effects of lepraline and mifepristone in the treatment of uterine fibroids.Methods A total of 120 patients with uterine fibroids were divided into observation group and control group according to random number table,with 60 cases in each group.The control group was treated with mifepristone,and the observation group was treated with lepraline acetate microspheres for injection.The levels of vascular growth factor(VEGF)and sex hormone,uterine volume,uterine fibroid volume,hemoglobin level,total effective rate and incidence of adverse reactions were compared between the two groups.Results Before treatment,there were no signifi cant differences in VEGF and sex hormone levels between the two groups(P>0.05).After treatment,the VEGF level in observation group was lower than that in control group,and the levels of luteinizing hormone(LH),estradiol(E_(2))and follicle-stimulating hormone(FSH)were lower than those in control group,with statistical signifi cance(P<0.05).Before treatment,there were no signifi cant differences in hemoglobin level,uterine volume and uterine fibroid volume between the two groups(P>0.05).After treatment,hemoglobin in observation group was higher than that in control group,uterine volume and uterine fi broid volume were lower than that in control group,the difference was statistically signifi cant(P<0.05).The total effective rate of the observation group was higher than that of the control group,the difference was statistically signifi cant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of uterine fibroids,lepraline can eff ectively regulate VEGF and sex hormone levels in patients with uterine fibroids.It has significant effect on reducing the volume and uterine fibroids,and helps to improve the hemoglobin level of patients.The therapeutic effect is better than mifepristone,and the safety is good.
