头孢噻肟钠舒巴坦钠在腹腔内感染中的应用价值

摘　　要：目的分析头孢噻肟钠舒巴坦钠在腹腔内感染中的应用价值。方法164例腹腔内感染患者,按照随机数字表法分为常规组、研究组,每组82例。常规组行常规抗感染治疗,研究组在常规组基础上加用头孢噻肟钠舒巴坦钠治疗。对比两组患者临床指标、临床疗效及治疗前1 d、治疗7 d、治疗结束后的生化指标[C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、Toll样受体(TLR)2 mRNA、TLR4 mRNA]。结果研究组患者体温恢复正常时间、住院时间分别为(2.13±0.50)、(17.35±2.09)d,短于常规组的(4.49±1.12)、(20.64±2.12)d,差异具有统计学意义(P<0.05)。常规组患者治疗前1 d、治疗7 d、治疗结束后CRP分别为(72.62±8.52)、(28.26±2.34)、(15.26±2.14)mg/L,PCT分别为(5.29±1.02)、(3.21±0.68)、(1.99±0.62)ng/ml,IL-6分别为(32.28±3.49)、(18.64±2.57)、(13.52±2.15)pg/ml,TNF-α分别为(36.79±4.26)、(23.67±2.19)、(13.64±1.98)pg/ml,TLR2 mRNA相对表达量分别为(2.85±0.23)、(2.09±0.33)、(1.72±0.19),TLR4 mRNA相对表达量分别为(2.61±0.41)、(1.85±0.23)、(1.16±0.24);研究组CRP分别为(72.68±8.19)、(19.68±2.22)、(8.29±1.02)mg/L,PCT分别为(5.34±1.19)、(2.06±0.25)、(0.78±0.21)ng/ml,IL-6分别为(32.52±3.55)、(12.25±2.19)、(6.49±1.02)pg/ml,TNF-α分别为(36.85±4.15)、(16.63±2.79)、(7.25±1.05)pg/ml,TLR2 mRNA相对表达量分别为(2.89±0.31)、(1.68±0.24)、(1.02±0.15),TLR4 mRNA相对表达量分别为(2.57±0.36)、(1.20±0.25)、(0.82±0.12);治疗7 d、治疗结束后,两组CRP、PCT、IL-6、TNF-α、TLR2 mRNA、TLR4 mRNA相对表达量均较本组治疗前1 d降低,且研究组降低程度显著大于对照组,差异具有统计学意义(P<0.05)。研究组治疗总有效率为95.12%,高于常规组的80.49%,差异具有统计学意义(P<0.05)。结论头孢噻肟钠舒巴坦钠对腹腔内感染的治疗有效,可明显改善临床症状,应用价值理想。Objective To analyze the application value of cefotaxime sodium and sulbactam sodium in intraperitoneal infection.Methods A total of 164 patients with intraperitoneal infection were divided into the conventional group and the research group according to the random numerical table,with 82 cases in each group.The conventional group was treated with conventional anti-infection therapy,and the research group was treated with cefotaxime sodium and sulbactam sodium on the basis of the conventional group.Both groups were compared in terms of clinical indexes,clinical efficacy and biochemical indexes[C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),Toll-like receptor(TLR)2 mRNA,TLR4 mRNA]1 d before treatment,7 d after treatment and at the end of treatment.Results The recovery time of body temperature and hospital stay in the research group were(2.13±0.50)and(17.35±2.09)d,which were shorter than(4.49±1.12)and(20.64±2.12)d in the conventional group,and the differences were statistically significant(P<0.05).In the conventional group,the CRP were(72.62±8.52),(28.26±2.34)and(15.26±2.14)mg/L 1 d before treatment,7 d after treatment and at the end of treatment;the PCT were(5.29±1.02),(3.21±0.68),(1.99±0.62)ng/ml;the IL-6 were(32.28±3.49),(18.64±2.57),(13.52±2.15)pg/ml;the TNF-αwere(36.79±4.26),(23.67±2.19),(13.64±1.98)pg/ml;the TLR2 mRNA relative expression were(2.85±0.23),(2.09±0.33),and(1.72±0.19);the TLR4 mRNA relative expression were(2.61±0.41),(1.85±0.23),(1.16±0.24).In the research group,the CRP were(72.68±8.19),(19.68±2.22),and(8.29±1.02)mg/L 1 d before treatment,7 d after treatment and at the end of treatment;the PCT were(5.34±1.19),(2.06±0.25),(0.78±0.21)ng/ml;the IL-6 were(32.52±3.55),(12.25±2.19),and(6.49±1.02)pg/ml;the TNF-αwere(36.85±4.15),(16.63±2.79),(7.25±1.05)pg/ml;the TLR2 mRNA relative expression were(2.89±0.31),(1.68±0.24),and(1.02±0.15);the TLR4 mRNA relative expression were(2.57±0.36),(1.20±0.25),(0.82±0.12).7 d after
