某院麻醉科麻醉药品使用情况分析

摘　　要：目的分析本院两年内麻醉科住院患儿手术种类、麻醉药品使用量、消耗金额变化趋势,探讨麻醉药品消耗金额异动的原因和合理性,为进一步加强麻醉科麻醉药品的合理使用和管理提供参考依据。方法收集本院麻醉科住院患儿手术类型、手术量、麻醉药品使用品种、数量、金额等临床资料。分析两年内本院麻醉药品使用的变化趋势,评估麻醉药品使用的潜在问题。结果本院麻醉科2020、2021年麻醉手术量分别为4683、4786台。增幅较大的前3类手术为斜视手术、包皮环切术、骨折内固定/固定物取出术。降幅较大的前3类手术为睾丸下降固定术、尿道下裂修补术、腹股沟斜疝/直疝修补术。2020年本院注射用瑞芬太尼、氢吗啡酮注射液、舒芬太尼注射液用量为124、1325、1868支,2021年升至673、2370、2061支,增幅达442.74%、78.87%、10.33%。氢吗啡酮注射液用量增幅前3类手术为斜视手术、腹腔镜下鞘状突高位结扎术、扁桃体/腺样体切除术,增幅分别达450.00%、119.05%、77.03%;舒芬太尼注射液用量增幅前3类手术为斜视手术、腹腔镜下阑尾切除术、腹腔镜下鞘状突高位结扎术,增幅分别达80.00%、51.72%、30.21%;注射用瑞芬太尼用量增幅前3类手术为腹腔镜下阑尾切除术、扁桃体/腺样体切除术、腹腔镜下鞘状突高位结扎术,增幅分别达3700.00%、2233.33%、1000.00%。2020年本院芬太尼注射液、吗啡注射液用量为674、592支,2021年降至362、5支,下降46.29%、99.16%。2020、2021年麻醉药品的消耗总金额分别为222409.3、367506.5元。2021年,注射用瑞芬太尼消耗金额由2020年的11147.6元增至60502.7元,增幅达442.74%;氢吗啡酮注射液、舒芬太尼注射液消耗金额由2020年的113453.1、92933.0元,增至202931.3、102534.8元,增幅达78.87%、10.33%;吗啡注射液消耗金额出现大幅度减少,由2020年的2044.8元降至17.3元,下降99.15%;芬太尼注射液�Objective To analyze the trend of the types of surgery,the amount of narcotic drugs and the amount of consumption in children hospitalized in the Department of Anesthesiology in the past two years,and explore the reasons and rationality of the variation of the amount of narcotic drugs consumption,so as to provide a reference for further strengthening the rational use and management of narcotic drugs in the Department of Anesthesiology.Methods The clinical data of the inpatients in the Department of Anesthesiology of our hospital were collected,such as the type of surgery,the amount of surgery,the variety,quantity and amount of narcotic drugs used.The change trend of narcotic drug used in our hospital in the past two years was analyzed to evaluate the potential problems of narcotic drug use.Results The number of anesthesia surgery in the Department of Anesthesiology of our hospital in 2020 and 2021 were 4683 and 4786 surgeries,respectively.The top 3 procedures with the largest increase were strabismus surgery,circumcision,and fracture internal fixation/fixation removal.The top 3 procedures with the greatest decrease were orchiopexy,hypospadias repair,and inguinal hernia/direct hernia repair.In 2020,the dosage of remifentanil for injection,hydromorphone injection and sufentanil injection in our hospital were 124,1325 and 1868 pieces,which increased to 6732370 and 2061 pieces in 2021,with an increase of 442.74%,78.87%and 10.33%.The top three increases in the dosage of hydromorphone injection were strabismus surgery,laparoscopic high ligation of the sheath process,tonsillectomy/adenoidectomy,with an increase of 450.00%,119.05%and 77.03%respectively;the top three increases in the dosage of sufentanil injection were strabismus surgery,laparoscopic appendectomy,and laparoscopic high ligation of the sheath process,with an increase of 80.00%,51.72%,and 30.21%respectively;the top three increases in the amount of remifentanil for injection were laparoscopic appendectomy,tonsillectomy/adenoidectomy,and laparoscopic high liga
