摘 要:目的:探讨鼻咽癌患者外周血中T细胞表面程序性死亡分子1(PD-1)在鼻咽癌发生、发展中的意义。方法:选取2018年2-12月中山市人民医院收治的50例鼻咽癌患者作为鼻咽癌组,选取同期50例在康体保健中心体检的人员作为对照组。比较两组外周血中T细胞表面PD-1、穿孔素和颗粒酶的表达情况。结果:鼻咽癌组CD4^(+)T、CD8^(+)T淋巴细胞的PD-1阳性表达率均高于对照组,差异有统计学意义(P<0.05);PD-1^(+)的CD8^(+)T细胞表达穿孔素和颗粒酶的阳性率低于PD-1^(-)的CD8^(+)T细胞,差异有统计学意义(P<0.05)。结论:PD-1与配体PD-L1相结合,可诱导T细胞丧失功能,PD-1^(+)的CD8^(+)T细胞表达穿孔素和颗粒酶的阳性率降低,这可能成为鼻咽癌治疗的新思路,值得进一步探索,PD-1有望成为治疗鼻咽癌的新靶点。Objective:To explore the significance of programmed death-1(PD-1)on T cell surface in peripheral blood of patients with nasopharyngeal carcinoma on the occurrence and development of nasopharyngeal carcinoma.Methods:Fifty patients with nasopharyngeal carcinoma admitted to Zhongshan City People’s Hospital from February 2018 to December 2018 were selected as the nasopharyngeal carcinoma group,and 50 people receiving physical examination in the Health Care Center during the same period were selected as the control group.The expressions of PD-1,perforin and granzyme on the surface of T cells in peripheral blood were compared between two groups.Results:The positive expression rate of PD-1 on CD4^(+)T and CD8^(+)T lymphocytes in nasopharyngeal carcinoma group was higher than that in control group,and the difference was statistically significant(P<0.05).The positive expression rate of perforin and granzyme on PD-1^(+) and CD8^(+)T cells was lower than that in PD-1^(-) and CD8^(+)T cells,and the difference was statistically significant(P<0.05).Conclusion:The combination of PD-1 and ligand PD-L1 can induce T cell loss function,and the positive expression rate of perforin and granzyme in PD-1^(+) and CD8^(+)T cells is reduced,which can provide a new idea for the treatment of nasopharyngeal carcinoma and deserve further exploration.PD-1 is expected to become a new target for the treatment of nasopharyngeal carcinoma.
