翼状胬肉切除联合自体角膜缘干细胞移植术临床疗效观察

摘　　要：目的:探讨翼状胬肉切除联合自体角膜缘干细胞移植术临床疗效。方法:2018年1月至2020年6月112例翼状胬肉患者随机分观察组及对照组各56例,分别采用单纯翼状胬肉切除术及自体角膜缘干细胞结膜瓣移植手术,观察指标:手术指标;治疗效果;术后半年选择激光干涉条纹对术眼散光程度和视力进行检测。结果:观察组结膜充血水肿缓解(1.15±0.22)d、植片充血水肿缓解(4.04±1.35)d、角膜创面修复(4.46±1.27)d、眼部刺激征消退(5.98±1.09)d短于对照组的(2.09±0.34)d、(5.24±1.40)d、(6.07±1.31)d、(7.22±1.76)d(P<0.05)。治疗前视力(4.49±0.37)vs(4.47±0.40)、散光度(1.31±0.53)D vs(1.27±0.49)D无差异(P>0.05),治疗6月以后,观察组裸眼视力(4.94±0.29)高于对照组的(4.53±0.35),角膜散光度(0.79±0.37)低于对照组的(1.10±0.41)(P<0.05)。观察组显效89.29%、好转8.93%、无效1.79%;对照组分别为60.71%、26.79%、12.50%;观察组总有效98.21%高于对照组87.50%(P<0.05),观察组复发1.79%低于对照组14.29%(P<0.05)。结论:对翼状胬肉患者进行自体角膜缘上皮移植术治疗,自体角膜缘结膜移植术中材料来源于自体,手术操作简捷、安全、经济,整体效果明显,降低了手术后复发率。Objective:To investigate clinical effect of pterygium excision combined with autologous limbal stem cell transplantation.Methods:Pterygium patients 112 cases were divided into observation and control group(n=56),Carry out pterygium excision and autologous limbal stem cell conjunctival flap transplantation for treatmen.Observation index:operation index;Therapeutic effect;Half a year after operation,the astigmatism and visual acuity were measured by laser interference fringe.Results:Observation group Conjunctival hyperemia and edema relieved for(1.15±0.22)days.graft congestion and edema relieved for(4.04±1.35)days.corneal wound repaired for(4.46±1.27)days,regression of ocular irritation sign(5.98±1.09)days was shorter than control group(2.09±0.34),(5.24±1.40),(6.07±1.31),(7.22±1.76)(P<0.05).There was no difference in visual acuity(4.49±0.37)vs(4.47±0.40)and astigmatism(1.31±0.53)d vs(1.27±0.49)d before treatment(P>0.05).After 6 months of treatment,the naked visual acuity(4.94±0.29)was higher than control group(4.53±0.35),and the corneal astigmatism(0.79±0.37)was lower than control group(1.10±0.41)(P<0.05).total effective higher than control group,recurrence rate 1.79%lower than control group 14.29%(P<0.05).Conclusion.The patients with pterygium were treated with autologous limbal epithelial transplantation.The materials of autologous limbal conjunctival transplantation came from the body.The operation was simple,safe and economical,and the overall effect was obvious,which reduced the recurrence rate after operation.
