摘  要:目的 生物力学比较切开与非切开软组织处理方式对正畸微种植体稳定性的影响。方法 60枚正畸微种植体(型号HAZ06),植入30名正畸患者下颌骨内,左侧下颌骨作为非切开组,右侧下颌骨作为切开组。非切开组微种植体的植入是不切开黏骨膜,植入时直接穿透黏骨膜自攻植入,即刻测量最高植入转矩。切开组是在植入点切开黏骨膜,将微种植体用自攻种植法植入到颌骨内,测量最高植入转矩,2周后切开组与非切开组加载200 g水平力。负载24周后检查微种植体的稳定性,并测量最高旋出转矩。比较两组微种植体的生物力学差异,有统计学意义(P<0.05)。同时,检验植入转矩和去除转矩是否存在相关性。结果 (1)60枚微种植体均成功植入下颌骨内,临床检查稳定性良好。(2)24周后临床成功率为:非切开组2枚种植体2级松动,2枚1级松动,成功率86.7%;切开组2枚种植体1级松动,成功率93.3%,两组比较差异有统计学意义(P<0.05)。(3)最高植入转矩比较:非切开组(8.81±3.25)N cm,切开组(8.73±3.32)N cm,差异无统计学意义(P>0.05);去除转矩比较:非切开组(4.67±3.39)N cm,切开组(5.23±3.76)N cm,差异有统计学意义(P<0.05)。非切开组的最高植入转矩和最高去除转矩有相关性(r=0.823,P<0.05),切开组的最高植入转矩和最高去除转矩有相关性(r=0.864,P<0.05)。结论 微种植体用作暂时性支抗,建议植入前的软组织处理方式为切开。Objective To compare the biomechanical effects of soft tissue incision and non-incision on the stability of orthodontic microimplants.Methods 60 orthodontic microimplants(model HAZ06) were implanted into mandible of 30 orthodontic patients.The right mandible was used as the open group and the left mandible as the non-open group.In the incision group, the mucperiosteum was cut at the implantation point to expose the bone surface, and the microimplants were implanted into the jaw by self-tapping implantation method, and the highest implant torque was measured immediately.In the non-open group, microimplants were implanted without cutting through the mucoperiosteal membrane, and the highest implant torque was measured. Two weeks later, 200 g horizontal force was applied to the open group and the non-open group.After 24 weeks of loading, the stability of the microimplants was checked and the maximum spin-out torque was measured.Paired T test was used to compare the biomechanical differences between the two groups, and there was statistical significance(P<0.05).Pearson test was used to evaluate the correlation between embedded torque and removed torque.Results(1) 60 microimplants were successfully implanted with good initial stability.(2)Clinical success rate after 24 weeks: in the non-incision group, 2 implants were grade 2 loose and 2 implants were grade 1 loose, with a success rate of 86.7%.(3)Comparison of maximum implant torque: non-incision group(8.81±3.25) N cm, incision group(8.73±3.32) N cm, no statistical significance(P>0.05).Torque removal comparison: non-incision group(4.67±3.39)N cm, incision group(5.23±3.76) cm, the difference was statistically significant(P<0.05).There was a correlation between the highest implanted torque and the highest removal torque in the non-incision group(r=0.823), and there was a correlation between the highest implanted torque and the highest removal torque in the incision group(r=0.864,P<0.05).Conclusion Microimplants are used as temporary anchorage, and incision is recomm
