摘　　要：目的探讨7项肺癌自身抗体(p53、SOX2、GAGE7、PGP9.5、GBU4-5、CAGE、MAGE A1)联合3项肿瘤标志物(CEA、CYFRA21-1、NSE)诊断早期肺癌的临床价值。方法选取2018年7月—2022年5月在宁波市医疗中心李惠利医院就诊的肺小结节患者708例,分为早期肺癌组患者543例、肺部良性疾病组患者165例,144例健康体检者作为健康对照组。利用ELISA法测定7项肺癌自身抗体水平,利用化学发光法测定肿瘤标志物水平。绘制工作特征曲线(ROC),分析7项肺癌自身抗体联合3项肿瘤标志物对早期肺癌患者的诊断效能。结果7项肺癌自身抗体联合3项肿瘤标志物(CEA、CYFRA21-1、NSE)的敏感度为40.88%,特异度为85.44%,准确度为57.04%,AUC为0.747,其敏感度、准确度和AUC均高于单项检测、肿瘤标志物联合及7项肺癌自身抗体联合。结论7项肺癌自身抗体联合3项肿瘤标志物(CEA、CYFRA21-1、NSE)在早期肺癌的诊断中有较高的临床参考价值。Objective To explore the clinical value of seven lung cancer autoantibodies(p53,SOX2,GAGE7,PGP9.5,GBU4-5,CAGE,MAGE A1)combined with three tumor markers(CEA,CYFRA21-1,NSE)in the diagnosis of early lung cancer.Methods A total of 708 patients with small pulmonary nodules were selected into this study,who accepted treatment in Ningbo Medical Center Lihuili Hospital from July 2018 to May 2022,including 543 cases of early lung cancer and 165 cases of benign lung disease.Meanwhile,144 patients who accepted physical examination in the hospital were collected as the control group.The concentrations of seven lung cancer autoantibodies were determined by enzyme linked immunosorbent assay(ELISA),and the levels of three tumor markers were determined by chemical luminescence.The ROC working curve were plotted to analyze the diagnostic efficiency of the seven lung cancer autoantibodies combined with three tumor markers in early lung cancer.Results The sensitivity,specificity,accuracy and AUC of the seven lung cancer autoantibodies combined with three tumor markers(CEA,CYFRA21-1,NSE)were 40.88%,85.44%,57.04%,0.747.Its sensitivity,accuracy and AUC were higher than those of single detection,tumor markers-combined detection and seven lung cancer autoantibodies-combined detection.Conclusion Seven lung cancer autoantibodies combined with three tumor markers(CEA,CYFRA21-1,NSE)have high clinical reference value in the diagnosis of early lung cancer.
