联合检测癌胚抗原、γ-谷氨酰基转移酶、乳酸脱氢酶对结直肠癌远处转移的诊断价值

摘　　要：目的 探讨CEA、LDH和GGT水平对结直肠癌远处转移情况的诊断价值。方法 对230例结直肠癌远处转移患者治疗前CEA、LDH和GGT的水平进行回顾性分析,比较结直肠癌远处转移与未转移患者CEA、GGT、LDH水平,利用受试者工作特征曲线计算以上血清学指标的最佳临界值,进一步比较肝内外转移的相应水平。结果 结直肠癌远处转移患者CEA、GGT和LDH水平高于未转移者(P<0.000 1)。CEA、LDH和GGT诊断结直肠癌远处转移的最佳截断值分别为8.225 ng/ml、27.50 U/L、209.50 U/L,联合诊断的ROC曲线下面积为0.822,诊断敏感度提升至0.847。肝转移与肝外转移相比,GGT和LDH水平差异有统计学意义,而CEA差异无统计学意义(P>0.05)。结论 将CEA、GGT和LDH联合检测可提高结直肠癌远处转移的诊断效能,评估转移位置,对决定治疗方案、改善患者预后具有积极作用。Objective To investigate the diagnostic value of carcinoembryonic antigen(CEA),lactate dehydrogenase(LDH)andγ-glutamyl transpeptidase(GGT)levels in the distant metastasis of colorectal cancer.Methods The pre-treatment CEA,LDH and GGT levels of 230 colorectal cancer patients with distant metastasis were retrospectively analyzed.The optimal critical values of the above serological indexes were calculated by receiver operating characteristic curve,and the corresponding levels of intrahepatic and extrahepatic metastasis were further compared.Results The levels of CEA,GGT and LDH in patients with distant metastasis of colorectal cancer are higher than those without metastasis(P<0.0001).The optimal cut-off values of CEA,LDH and GGT for the diagnosis of distant metastasis of colorectal cancer are 8.225 ng/ml,27.50 U/L and 209.50 U/L,respectively.The area under the ROC curve of the combined diagnosis is 0.822,and the diagnostic sensitivity was improved to 0.847.Compared with extrahepatic metastases,the level differences of GGT and LDH in liver metastases are statistically significant,but there is no statistical significance on the difference in CEA(P>0.05).Conclusion The combined detection of CEA,GGT and LDH can improve the diagnostic efficiency of colorectal cancer’s distant metastasis and evaluate the location of metastasis.It will play a positive role in selecting the treatment plans and improving the prognosis of patients.
