摘  要:目的探讨脑源性神经营养因子(BDNF)和胰岛素样生长因子-1(IGF-1)水平与脑小血管病所致认知功能障碍的关系。方法选择2019年3月—2021年12月本院收治的99例脑小血管病患者作为研究对象,应用蒙特利尔认知评估量表对其认知功能进行评估,将患者分为认知功能障碍组49例和认知功能正常组50例,比较2组患者临床资料和血清学指标,应用logistic回归分析影响认知功能障碍的因素,应用Pearson相关性分析BDNF、IGF-1水平与MoCA评分的关系。结果认知功能障碍组患者高血压比例、糖尿病比例高于认知功能正常组(P<0.05);认知功能障碍组患者Hcy水平高于认知功能正常组(P<0.05),BDNF、IGF-1水平低于认知功能正常组(P<0.05);高血压、糖尿病、BDNF、IGF-1是影响CSVD患者认知功能障碍的主要因素(P<0.05),Hcy与认知功能障碍无关(P>0.05);重度组患者BDNF、IGF-1水平低于中度组、轻度组和正常组(P<0.05),中度组BDNF、IGF-1水平低于轻度组和正常组(P<0.05),且轻度组BDNF、IGF-1水平低于正常组(P<0.05);BDNF、IGF-1水平与CSVD患者MoCA评分成正相关(P<0.05)。结论BDNF、IGF-1水平与CSVD所致认知功能障碍关系密切,二者水平越低,患者认知功能障碍越严重。Objective To investigate the relationship between the levels of brain-derived neurotrophic factor(BDNF)and insulin-like growth factor-1(IGF-1)and cognitive dysfunction caused by cerebral small vessel disease.Methods A total of 99 patients with cerebral small vessel disease admitted to our hospital from March 2019 to December 2021 were selected as subjects,and the cognitive functions were assessed by the Montreal Cognitive Assessment MoCA Scale.The patients were divided into cognitive impairment group(n=49)and normal cognitive function group(n=50),and the clinical data and serological indicators of the two groups were compared.Logistic regression was used to analyze the factors influencing cognitive dysfunction,and Pearson correlation was used to analyze the relationship between BDNF,IGF-1 levels and MoCA score.Results The proportion of hypertension and diabetes mellitus in cognitive impairment group was higher than that in normal cognitive function group(P<0.05).Hcy level in the cognitive impairment group was higher than that in the normal cognitive function group(P<0.05),and the levels of BDNF and IGF-1 were lower than those of normal cognitive function group(P<0.05).Hypertension,diabetes mellitus,BDNF and IGF-1 are the main factors affecting cognitive dysfunction of CSVD(P<0.05),Hcy is not significantly associated with cognitive dysfunction(P>0.05).The levels of BDNF and IGF-1 in severe group were lower than those in moderate group,mild group and normal group(P<0.05),the levels of BDNF and IGF-1 in moderate group were lower than those in mild group and normal group(P<0.05),and the levels of BDNF and IGF-1 in mild group were lower than those in normal group(P<0.05).BDNF and IGF-1 was positively correlated with MoCA score(P<0.05).Conclusion The levels of BDNF and IGF-1 are closely related to the cognitive dysfunction caused by cerebral small vessel disease,and the lower the level of BDNF and IGF-1,the more serious the cognitive dysfunction.
