摘　　要：目的 探讨数字乳腺断层合成摄影(DBT)联合全屏数字化乳腺X线摄影(FFDM)及超声在乳腺结构扭曲良恶性病变的影像征象,分析结构扭曲病变的诊断价值.方法 选取术前行乳腺FFDM、DBT、超声且表现为结构扭曲、有病理结果的病例154例,对结构扭曲良恶性病变征象及不同检查方式诊断效能行统计学分析.结果乳腺FFDM联合DBT良恶性病变在结构扭曲形态、结构扭曲中心密度有统计学差异;乳腺超声良恶性病变在病变边缘、后方回声、钙化、血流有统计学差异(P<0.05).FFDM联合DBT敏感度及特异度均大于FFDM、超声,约登指数最高,三种检查方式的受试者工作特征(ROC)曲线对比,差异有统计学意义(P<0.05).结论 通过结构扭曲病变不同影像征象分析,进一步判断及分析病变的良恶性;使用FFDM联合DBT结构扭曲病变的检出率提高.Objective To investigate digital breast tomosynthesis(DBT)combined with full-field digital mammography(FFDM)and ultrasonography in benign and malignant lesions of breast with distorted structure,and to analyze the diagnostic value of structural distortion.Methods A total of 154 breast patients who underwent FFDM and DBT ultrasonography before surgery and showed structural distortion and pathological results were selected.The signs of benign and malignant lesions with structural distortion and the diagnostic efficiency of different examination methods were statistically analyzed.Results There were statistically significant differences between benign and malignant breast FFDM combined with DBT in structural distortion morphology,center density of structural distortion.There were statistical differences between benign and malignant lesions of breast ultrasound in lesion margin,posterior echo,calcification and blood flow(P<0.05).The sensitivity and specificity of FFDM combined with DBT were greater than FFDM and ultrasound,and the Jorden index was the highest.ROC curves of the three methods were compared,and the difference was statistically significant(P<0.05).Conclusion Through the analysis of different image signs of structural distortion lesions,the benign and malignant lesions were further judged and analyzed.The detection rate of FFDM combined with DBT was improved.
