摘 要:目的 探究超级迭代PET/CT定量技术对肺癌化疗疗效的预测价值.方法 搜集本院108例肺癌化疗患者,根据疗效分为缓解组与未缓解组,行超级迭代PET/CT定量技术检查.结果 缓解组治疗前、治疗2个周期后平均标准摄取值(SUV_(mean))、标准摄取值峰值(SUV_(peak))、肿瘤代谢体积(MTV)、病灶糖酵解总量(TLG)低于未缓解组,治疗前与治疗2个周期后SUV_(mean)、SUV_(peak)、MTV﹑TLG差值高于未缓解组(P<0.05);校正其他因素前后,SUV_(mean) ﹑SUV_(peak)、MTV、TLG差值均与肺癌化疗疗效独立相关(P<0.05);SUV_(mean)、SUV_(peak)、MTV、TLG差值联合预测肺癌化疗获得缓解的曲线下面积(AUC)为0.889(95%CI:0.814~0.941),大于各参数差值单独预测;以截断值为界,SUV_(mean)、SUV_(peak)、MTV、TLG差值低水平患者1年内死亡危险度分别是高水平患者的5.274倍、3.750倍、3.182倍、3.500倍.结论 超级迭代PET/CT定量技术参数在肺癌化疗前后的变化差值联合预测获得缓解的效能较高,且对患者1年内死亡危险度有提示作用.Objective 108 patients with lung cancer conservatively treated in our hospital were selected and divided into remission group and non-remission group according to the curative effect.Super iterative PET/CT quantitative technology examination was performed.The results showed that the remission group was SUV_(mean),SUV_(peak),SUV_(peak) before treatment and after 2 cycles of treatment.MTV and TLG were lower than those in the non-remission group,and the difference between SUV_(mean),SUV_(peak),MTV and TLG before treatment and after 2 cycles of treatment was higher than that in the non-remission group(P<0.05);Before and after adjustment for other factors,the differences between SUV_(mean),SUV_(peak),MTV,and TLG were independently correlated with the efficacy of conservative treatment of lung cancer(P<0.05);the difference between SUV_(mean),SUV_(peak),MTV,and TLG combined to predict the remission of conservative treatment of lung cancer had an AUC of 0.889(95%CI:0.814-0.941),greater than the difference of each parameter separately predicted;using the cutoff value,the risk of death within 1 year of patients with low SUV_(mean),SUV_(peak),MTV,and TLG differences is 5.274 times and 3.750 times that of high-level patients,respectively,3.182 times,3.500 times.It is suggested that the super-iterative PET/CT quantitative technical parameters before and after conservative treatment of lung cancer are more effective in predicting remission,and it has a prompting effect on the patient’s death risk within 1 year.
