摘  要:目的 探讨平板DSA三维旋转血管造影(3DRA)技术在颈动脉支架置入术(CAS)中的应用价值.方法选取缺血性脑卒中颈动脉狭窄患者40例,所有患者术前均行DSA介入检查并行术中3DRA,3DRA原始数据传输至Philips Allura X-per DSA X-travision 3D后处理工作站,应用彩色容积重组技术进行图像后处理,通过3DRA图像结合三维后处理彩色容积图像对颈动脉狭窄病变进行分析评估,并给予医师最佳的手术工作角度,引导手术入路,进行支架置入术.结果 40例颈动脉狭窄患者(42支血管)中,颈内动脉重度狭窄24支,颈内动脉中度狭窄14支(其中2例为双侧颈内动脉中度狭窄),颈内动脉轻度狭窄4支,均通过3DRA技术明确诊断,并指导手术入路顺利置入颈动脉支架33枚.结论 平板DSA-3DRA技术对颈内动脉狭窄病变程度及狭窄斑块的形态学分析与评估优于2D-DSA,准确显示狭窄病变形态,精准指导手术入路,有助于手术顺利进行,降低CAS中并发症的发生率及减少术中辐射剂量.Objective To investigate the application value of three-dimensional rotation angiography(3 DRA)technology of panel DSA in internal carotid artery stenting.Methods 40 cases of patients with internal carotid artery stenosis admitted in our center underwent carotid artery stenting.DSA-3 DRA was performed before the operation.The 3 DRA raw data was transferred to Philips Allura X-per DSA Xtravision 3 D workstation,and the color volume reconstruction technology was applied.According to 3 DRA images and three-dimensional color volume reconstruction images,the doctor analyzed and evaluated carotid artery stenosis lesions,then offered the best operative position,guiding the approach of surgery,and performing stent implantation.Results In 40 cases with carotid artery stenosis,there were 24 patients with severe internal carotid artery stenosis,14 patients with moderate internal carotid artery stenosis(two of them were bilateral carotid artery moderate stenosis),and 4 patients had mild internal carotid artery stenosis.All of them were confirmed diagnosis by 3 DRA,the approach of surgery was planned,then the doctor successfully placed 33 carotid stents.Conclusion The DSA-3 DRA technology is superior to 2 D-DSA in analyzing and evaluating the grading of carotid artery stenosis and the morphological of Carotid artery plaques.3 DRA can accurately display the stenosis and precisely guide the approach of surgery,not only makes sure the operation to proceed safely,but also reduces The incidence of complications and radiation dose during CAS operation.
