摘 要:目的探讨多模态超声特征对乳腺癌上皮型钙黏蛋白(E-cadherin)、钙调蛋白(calponin)和细胞角蛋白5/6(CK5/6)表达的预测价值。方法术前对86例乳腺癌患者病灶进行常规超声和超声造影检查,采用免疫组织化学法检测病灶中E-cadherin、calponin和CK5/6的表达。应用单因素分析和二元logistic回归模型分析多模态超声特征与乳腺癌中E-cadherin、calponin和CK5/6表达的关系。结果单因素分析结果显示,常规超声中肿块边缘状态和血流分级与E-cadherin表达有关,病灶内有无微钙化与calponin和CK5/6表达有关(P<0.05);超声造影中增强后范围与calponin表达有关(P<0.05)。二元logistic回归分析结果显示,肿块边缘光整和血流分级高级别是乳腺癌中E-cadherin阳性表达的独立预测因素(P<0.05);病灶内无微钙化和增强后范围增大是calponin阳性表达的独立预测因素(P<0.05)。结论多模态超声特征对乳腺癌E-cadherin、calponin和CK5/6的表达具有良好的预测价值,有助于辅助乳腺癌的临床诊疗。Objective To investigate the predictive value of multimodal ultrasound characteristics in the expressions of E-cadherin,calponin and cytokeratin 5/6(CK5/6)in breast cancer.Methods Before operation,86 breast cancer patients were examined by conventional ultrasound and contrast-enhanced ultrasonography(CEUS).The expressions of E-cadherin,calponin and CK5/6 in breast cancer were detected by immunohistochemistry.Binary logistic regression was applied to analyze the correlation between multimodal ultrasound characteristics and the expressions of E-cadherin,calponin and CK5/6 in breast cancer.Results The results of univariate analysis showed that the tumor margin state and blood flow grade were related to the expression of E-cadherin,and microcalcification was related to the expressions of calponin and CK5/6 in conventional ultrasound(P<0.05),and the range after enhancement was related to the expression of calponin in CEUS(P<0.05).The results of binary logistic regression analysis showed that the smooth margin of tumor and the high blood flow grade were the independent predictor factors for E-cadherin positive expression(P<0.05),absence of microcalcification and increased range after enhancement were the independent predictor factors for calponin positive expression(P<0.05).Conclusion Multimodal ultrasound characteristics have good predictive value for the expressions of E-cadherin,calponin and CK5/6 in breast cancer,which is helpful for clinical diagnosis and treatment of breast cancer.
