摘　　要：目的探讨甲状腺乳头状癌(PTC)患者血清氨肽酶P2(XPNPEP2)表达水平及临床意义。方法收集2021年1月至2022年9月间,华中科技大学附属同济医院甲乳外科收治的甲状腺肿瘤患者149例,收集其常规术前血液检验后剩余血清,酶联免疫吸附试验(ELISA)法检测血清中的XPNPEP2水平。以术后常规病理检查将纳入病例分为PTC组及甲状腺良性肿瘤(BT)组,比较两组XPNPEP2水平差异;同时在PTC组,分析XPNPEP2与年龄、性别、肿瘤大小、淋巴结转移等临床病理特征上的相关性。结果共纳入甲状腺肿瘤病例149例,其中良性肿瘤61例,PTC 88例;PTC患者血清XPNPEP2水平明显低于良性肿瘤患者[比值比(OR)=-60.27,P<0.01]。PTC病例中,侧方淋巴结转移患者的血清XPNPEP2水平明显高于侧方淋巴结无转移患者(OR=65.41,P<0.05),但与肿瘤大小、年龄、性别、促甲状腺激素、中央组淋巴结转移无明显相关。结论甲状腺乳头状癌血清XPNPEP2明显降低。血清XPNPEP2水平与PTC淋巴结转移明显相关。Objective To investigate the serum aminopeptidase P2(also called XPNPEP2)level of papillary thyroid carcinoma(PTC),and evaluate its clinical significance.Methods The patients’peripheral blood was colleted during june 2021 to January 2022 in Tongji Hospital wuhan.Using enzyme-linked immunosorbent assay(ELISA)to measure the serum levels of XPNPEP2.Benign thyroid tumor(BT)and PTC diagnosed postoperatively by biopsy was analyzed retrospectively.Statistical analysis was performed with serum XPNPEP2 level and the clinicopathologic features.Results 149 cases of thyroid tumor were analyzed,61 BT,88 PTC.The serum XPNPEP2 level of PTC was significanly lower than that in BT[odds ratio(OR)=-60.27,P<0.01].Multi-factor regression analysis showed that the serum XPNPEP2 Level was an independent indicator for lateral cervical lymph nodes metastasis in PTC(OR=65.41,P<0.05).Conclusion The expression level of XPNPEP2 in peripheral blood of PTC patients is decressed.The level of XPNPEP2 may an indicator of PTC with poor prognosis.
