摘 要:目的探讨尿液蛋白激酶Y(PRKY)基因甲基化联合前列腺影像数据与报告系统(PI-RADS)评分在前列腺癌(PCa)中的诊断价值。方法收集2018年10月至2019年10月在苏州大学附属第二医院住院的51例可疑PCa患者的尿液,提取DNA后通过焦磷酸测序法检测PRKY基因启动子区甲基化水平,同时根据前列腺穿刺活检病理结果将患者分为PCa组和良性前列腺增生(BPH)组。采用Mann-Whitney检验分析两组患者年龄、前列腺特异性抗原(PSA)等临床指标的差异,运用χ^(2)检验分析两组患者尿液中的PRKY基因启动子甲基化状态。建立PRKY甲基化和PI-RADS评分的受试者工作特征曲线(ROC),计算曲线下面积(AUC),分析PRKY甲基化在PCa中的诊断价值,并联合磁共振PI-RADS评分进行联合诊断。结果PCa和BPH患者尿液中PRKY甲基化阳性率分别为79.17%(19/24)和25.93%(7/27),差异有统计学意义(χ^(2)=3.516,P<0.01)。ROC分析得出PRKY甲基化对于诊断PCa的曲线下面积(AUC)为0.813,PRKY甲基化联合PI-RADS评分诊断PCa的AUC为0.982。结论尿液PRKY基因甲基化检测以及联合PI-RADS评分有望成为PCa新的无创诊断方式。Objective To investigate the diagnostic value of urinary protein kinase Y-linked(PRKY)gene methylation combined with prostate imaging reporting and data system(PI-RADS)score in prostate cancer(PCa).Methods From October 2018 to October 2019,urine samples were collected from 51 patients with suspected PCa who were hospitalized at Soochow University′s Second Affiliated Hospital.After DNA was extracted,the methylation level in the promoter region of PRKY gene was detected by pyrophosphate sequencing.According to the pathological results of prostate biopsy,the patients were divided into PCa group and benign prostatic hyperplasia(BPH)group.The two groups′differences in age,prostate specific antigen(PSA)levels,and other clinical indications were compared using the Mann-Whitney test,and the methylation status of the PRKY gene promoter in each group′s urine was also analyzed byχ^(2) test.Receiver operating characteristic curves(ROC)of PRKY gene methylation and PI-RADS score were established and the areas under the curve(AUC)were calculated.The diagnostic value of PRKY gene methylation in PCa was analyzed,and combined with magnetic resonance imaging PI-RADS score for joint diagnosis.Results The positive rate of PRKY gene methylation in urine of patients with PCa and BPH was 79.17%(19/24)and 25.93%(7/27)respectively,and the difference was statistically significant(χ^(2)=3.516,P<0.01).ROC analysis showed that the area under the curve(AUC)of PRKY methylation for the diagnosis of PCa was 0.813 and the AUC of PRKY methylation combined with PI-RADS score for the diagnosis of PCa was 0.982.Conclusion Urinary PRKY gene methylation combined with PI-RADS score is a promosing non-invasive strategy for the diagnosis of PCa.
