摘  要:目的构建一种小鼠颈部游离皮瓣移植模型,并初步探讨同种异体皮瓣移植排斥规律。方法选择MHC不相符的近交系BALB/c小鼠(第四军医大学动物实验中心)作为供体,以C57bl/6小鼠为受体,将小鼠利用随机数字表法分为3组,包括同基因移植组:C57bl/6→C57bl/6;急性排斥组:BALB/c→C57bl/6不用药;免疫抑制剂组:BALB/c→C57bl/6联合输注3 mg/(kg·d)雷帕霉素。供体腹股沟皮瓣以小鼠股动静脉为蒂,获取皮瓣后,将血管穿过套管,并将内膜翻折出并固定;受体区域选择小鼠颈部,在颈部去除皮肤,舌下腺等结构后暴露颈动脉和颈外静脉,利用血管套管技术,将皮瓣股动脉与颈动脉相接,将皮瓣股静脉与颈外静脉相接。观察皮瓣存活,病理染色检查血管通畅情况,生存资料采用Kaplan-Meier方法统计,组之间的差异采用Log-Rank法来比较。结果供体皮瓣获取时间为(43±3)min,总手术时间为(91±7)min,皮瓣整体缺血时间(73±2)min。同基因移植组皮瓣均获得长期存活,急性排斥组皮瓣在术后10 d左右均发生了排斥;免疫抑制组皮瓣中位生存时间高于急性排斥组皮瓣存活时间(80 d比10 d,χ^(2)=9.039,P<0.01)。结论通过血管吻合套管技术构建游离皮瓣移植模型,为临床血管化皮肤移植研究提供了较理想的动物模型。Objective To establish an experimental model of free skin flap in mouse and evaluate the rejection process of the skin allografts.Methods BALB/c and C57bl/6 mice were used as donors and recipients respectively.The mice were randomly divided into 3 groups.The syngeneic group,skin flaps were performed between identical C57bl/6 mice.The skin flaps were transplanted between BALB/c and C57bl/6 in the rejection and immunosuppression group.No treatment was performed in rejection group,whereas mice of immunosuppression group received 3 mg/(kg·d)rapamycin.Free groin skin flap of the donor pedicled by femoral vessels was raised and transplanted to the neck of the recipients.After removal of the cervical skin and submandibular gland of the recipient,the common carotid artery(CCA)and external jugular vein(EJV)were exposed.A nonsuture cuff technique was utilized to anastomose donor femoral artery to recipient CCA and donor femoral vein to recipient EJV.The skin flaps were monitored daily and histopathological evaluation was used to monitor vascular patency.Survival data were analyzed by Kaplan-Meier method,and differences between groups were compared by log-rank method.Results The flap harvest time was(43±3)min,the total operation time was(91±7)min,the flap ischemia time was(73±2)min.Long-term skin flap survival was achieved in syngeneic group;skin allografts were rejected in about 10 days without any treatment;the median survival time of immunosuppression group was longer than that of rejection group(80 days to 10 days,χ^(2)=9.039,P<0.01).Conclusion We successfully established a mouse free skin flap model by nonsuture cuff technique,which provided an ideal model for the research of vascularized skin allotransplantation.
