腮腺囊肿28例临床病理分析

摘　　要：目的:总结腮腺囊肿的临床病理表现及治疗效果。方法:回顾分析28例腮腺囊肿患者病历资料、临床表现、病理诊断以及手术疗效。结果:28例患者中男13例,女15例,年龄23~78岁,平均54.5岁。均发病在单侧腮腺,左侧11例、右侧17例;囊肿位于浅叶27例、深叶1例。潴留型囊肿9例,外渗性囊肿5例,腮腺导管囊肿1例,淋巴上皮囊肿12例,表皮样囊肿1例。7例行腮腺部分切除,20例行腮腺浅叶切除,1例行腮腺全叶切除。术后暂时性面神经麻痹1例,治疗后恢复;随访3个月至11年无复发。结论:腮腺囊肿缺乏特征性临床表现,组织病理学表现多样,依据囊肿类型选择手术方案,预后良好。Objective:To summarize the clinical and histopathological characteristics of parotid gland cyst.Methods:The medical history,clinical manifestations,pathological diagnosis and treatment effect of 28 patients with parotid gland cyst were retrospectively analyzed.Results:There were 15 females and 13 males,the age ranged from 23 to 78 years,with an average age of 54.5 years.All cases were unilateral,with 11 cases on the left and 17 cases on the right,27 cases in the superficial lobe and 1 case in the deep lobe.There were 9 cases of retention cyst,5 cases of extravasation cyst,1 case of parotid duct cyst,12 cases of benign lymphoepithelial cyst(also known as first branchial cleft cyst),and 1 case of epidermoid cyst.Partial parotidectomy was performed in 7 cases,superficial parotid lobectomy in 20 cases,and total parotid lobectomy in 1 case.There was 1 case of temporary facial paralysis after operation,and there was no recurrence during follow-up.Conclusion:The parotid gland cyst has no characteristic clinical feature and various histopathological manifestations.The surgical plan is selected according to the type of cyst,and the prognosis is good.
