乳腺癌组织中相关蛋白表达情况及其与临床病理特征和预后的关系

摘　　要：目的 探讨乳腺癌组织中葡萄糖转运蛋白-1(GLUT-1)、磷酸酶张力蛋白同源物(PTEN)、人类表皮生长因子受体-2(HER-2)、增殖细胞核抗原(PCNA)蛋白的表达情况,并分析其与乳腺癌患者临床病理特征及预后的关系,为临床诊治该疾病提供依据。方法 回顾性分析徐州市中心医院2012年1月至2017年1月收治的80例乳腺癌患者的临床资料,应用免疫组化法检测手术切除的肿瘤组织和癌旁正常组织中GLUT-1、PTEN、HER-2、PCNA蛋白表达情况。比较乳腺癌组织与癌旁正常组织GLUT-1、PTEN、HER-2、PCNA蛋白表达情况;不同临床病理特征的乳腺癌患者乳腺癌组织GLUT-1、PTEN、HER-2、PCNA蛋白表达情况;依据5年的随访结果将患者分为生存组(59例)、死亡组(21例),分析不同预后患者乳腺癌组织GLUT-1、PTEN、HER-2、PCNA蛋白表达情况。结果 与癌旁正常组织相比,乳腺癌组织中GLUT-1、HER-2、PCNA蛋白阳性表达率显著升高,PTEN蛋白表达阳性率显著降低(均P<0.05);临床分期为Ⅲ~Ⅳ期、淋巴结转移的乳腺癌患者乳腺癌组织中GLUT-1、HER-2、PCNA蛋白表达阳性率显著高于临床分期为Ⅰ~Ⅱ期、淋巴结未转移者,PTEN蛋白表达阳性率显著低于临床分期为Ⅰ~Ⅱ期、淋巴结未转移者;死亡组乳腺癌组织中GLUT-1、HER-2、PCNA蛋白阳性表达率显著高于生存组,PTEN蛋白阳性表达率显著低于生存组(均P<0.05)。不同年龄、肿瘤直径的乳腺癌患者乳腺癌组织中GLUT-1、PTEN、HER-2、PCNA蛋白表达阳性率经比较,差异均无统计学意义(均P>0.05)。结论 乳腺癌组织中GLUT-1、HER-2、PCNA蛋白呈现高表达,PTEN蛋白呈现低表达,并与其临床分期、淋巴结转移及预后情况关系密切,进行乳腺癌组织GLUT-1、PTEN、HER-2、PCNA蛋白表达的检测能够为乳腺癌的诊断与预后评估提供参考。Objective To investigate the expression of glucose transporter-1(GLUT-1), phosphatase and tensin homolog deleted on chromosome ten(PTEN), human epidermal growth factor receptor-2(HER-2) and proliferating cell nuclear antigen(PCNA) in breast cancer,and analyze its relationship with clinicopathological characteristics and prognosis of breast cancer patients, so as to provide basis for clinical diagnosis and treatment of the disease. Methods The clinical data of 80 patients with breast cancer admitted to Xuzhou Central Hospital from January 2012 to January 2017 were retrospectively analyzed, and the expression of GLUT-1, PTEN, HER-2 and PCNA proteins in tumor tissues and normal tissues adjacent to cancer were detected by immunohistochemical method. The expression of GLUT-1, PTEN, HER-2,PCNA protein in breast cancer tissues and normal tissues adjacent to cancer, and the expression of GLUT-1, PTEN, HER-2, PCNA protein in breast cancer tissues of patient with different clinicopathological characteristics were compared;the patients were divided into survival group(59 cases) and death group(21 cases) according to the 5-year follow-up results, the expression of GLUT-1, PTEN, HER-2 and PCNA proteins in breast cancer tissues of patients with different prognosis were analyzed. Results Compared with normal tissues adjacent to cancer, the positive rates of GLUT-1, HER-2 and PCNA protein expression in breast cancer tissues were significantly higher, and the positive rates of PTEN protein expression were significantly lower(all P<0.05);the positive rates of GLUT-1, HER-2 and PCNA protein expression in breast cancer tissue of patients with clinical stage Ⅲ~Ⅳ and lymph node metastasis were significantly higher than those of patients with clinical stage Ⅰ~Ⅱ and without lymph node metastasis, the positive rate of PTEN protein expression was significantly lower than that of patients in clinical stage Ⅰ~Ⅱ and without lymph node metastasis;the positive rates of GLUT-1, HER-2 and PCNA protein expression in breast cancer tiss
