摘　　要：目的探讨VAD治疗初诊多发性骨髓瘤(MM)患者的疗效及其影响因素。方法回顾性分析行VAD方案治疗的124例初诊MM患者临床资料,分析VAD治疗初诊MM患者的疗效及其影响因素。结果VAD治疗初诊MM患者治疗总有效率为65.32%(81/124),其中完全缓解(CR)20例(16.13%),部分缓解(PR)61例(49.19%),稳定(SD)27例(21.77%),疾病进展(PD)16例(12.90%)。按照治疗效果分为治疗有效组81例和治疗无效组43例,单因素分析显示:年龄、Durie-Salmon(DS)分期、C反应蛋白(CRP)、白蛋白、血肌酐、乳酸脱氢酶(LDH)、β_(2)微球蛋白(β_(2)-MG)与VAD治疗初诊MM疗效有关,差异有统计学意义(P<0.05);多因素分析显示:年龄≥60岁、DS分期Ⅲ期、CRP≥5 mg/L、白蛋白<35 g/L、血肌酐≥10.7 mmol/L、LDH≥240 U/L、β_(2)-MG≥3.5 mg/L是影响VAD治疗初诊MM疗效的高危因素(P<0.05)。结论VAD治疗初诊MM疗效较好,但疗效受年龄、DS分期、CRP、白蛋白、血肌酐、LDH、β_(2)-MG等因素影响,临床需加以重视。Objective To investigate the efficacy and influencing factors of VAD for patients with multiple myeloma(MM).Methods The clinical data of 124 primary MM patients treated with the VAD regimen were analyzed comprehensively, and the efficacy of VAD treatment in primary MM patients and its influencing factors were analyzed.Results The total response rate of naive MM patients was 65.32%(81/124),including 20 complete remission(CR)(16.13%(C R),61 partial remission(PR)(49.19%),stable(SD) 27(21.77%),and 16 disease progression(PD)(12.90%).According to the treatment effect, 81 patients were in the treatment effective group and 43 patients were in the treatment ineffective group, Univariate analysis showed that age, Durie-Salmon(DS) stage, C reactive protein(CRP),albumin, blood creatinine, lactate dehydrogenase(LDH),and β_(2)microglobulin(β_(2)-MG) were related to the efficacy of initial MM in the treatment of VAD,The difference was statistically significant(P<0.05);Multivariate analysis showed that ≥60 years, DS stage, CRP 5 mg/L,albumin <35 g/L,blood creatinine 10.7 mmol/L,LDH 240 U/L,and β_(2)-MG 3.5 mg/L were the high-risk factors affecting the efficacy of primary MM in VAD treatment(P<0.05).Conclusion VAD treatment in primary MM is very effective, but age, DS stage, CRP,albumin, blood creatinine, LDH,β_(2)-MG,etc are influencing factors.
