摘　　要：目的 探讨原发性肝癌(PLC)术后患者创伤后成长(PTG)状况及相关影响因素。方法 回顾性分析89例PLC患者的临床资料。应用创伤后成长量表(PTGI)评价所有患者创伤后成长情况,并依据PTGI评分将患者分为低水平组与中高水平组,收集两组年龄、性别、体质量指数、文化程度、病理分期、家庭人均月收入、婚姻状况、费用来源、工作状况、社会支持水平、居住地、放化疗等临床资料,先后开展单因素分析、Logistic回归分析,获得影响PLC术后患者PTG状况的独立危险因素。结果 89例患者PTGI评分为(61.25±5.19)分,整体处于中等水平,其中低水平为29例,中等水平为41例,高水平为19例;单因素分析显示,两组文化程度、家庭人均月收入、婚姻状况、社会支持水平、放化疗相比,差异有统计学意义(P<0.05);多因素分析显示,文化程度初中及以下、家庭人均月收入<5000元、丧偶或离异、社会支持水平低、放化疗为影响PLC术后患者PTG状况的独立危险因素(P<0.05且OR>1)。结论 PLC术后患者PTG处于中等水平,文化程度初中及以下、家庭人均月收入<5000元、丧偶或离异、社会支持水平低、放化疗为影响PLC术后患者PTG状况的独立危险因素,临床还需以此开展针对性心理干预,以改善患者PTG水平。Objective To investigate the post-traumatic growth(PTG) status and related influencing factors in patients after primary liver cancer(PLC).Methods Clinical data of 89 PLC patients were retrospectively analyzed.Post-traumatic growth scale(PTGI) to evaluate post-traumatic growth of all patients, and divided into low level and high level group according to PTGI score, collect age, gender, body quality index, cultural level, pathological stage, monthly income, marital status, cost source, work status, social support level, residence, chemotherapy clinical data, univariate analysis, Logistic regression analysis, obtain independent risk factors affecting PTG status after PLC patients.Results The PTGI score of 89 patients was(61.25±5.19),Overall, at a medium level, Of these, the low level was found in 29 cases, The moderate level was observed in 41 cases, High level was 19 cases;The univariate analysis revealed that, Compared with the two groups, per capita monthly income, marital status, social support level and chemo-chemotherapy, The difference was statistically significant(P<0.05);Multivariate analysis revealed that, Junior high school or below, a per capita monthly income <5,000 yuan, widowed or divorced, low level of social support, and chemoradiotherapy were independent risk factors affecting PTG status after PLC(P<0.05 and OR> 1).Conclusion PTG after PLC is at a medium level, with middle education or below, per capita monthly income of <5000 yuan, widowed or divorced, low level of social support, and radiotherapy and chemotherapy are independent risk factors affecting PTG after PLC,and targeted psychological intervention is needed to improve the PTG level.
