摘　　要：目的 探究早期结直肠癌及癌前病变内镜黏膜下剥离术(ESD)后迟发性出血发生的影响因素。方法 选择100例早期结直肠癌及癌前病变患者,患者均行ESD治疗。观察患者术后迟发性出血发生情况,并分析ESD治疗后迟发性出血发生的相关因素。结果 依据ESD治疗后是否出现迟发性出血分为出血组(11例)及未出血组(89例);出血组直肠病变、重度纤维化发生率均高于未出血组,有统计学差异(P<0.05);Logistic回归分析显示,病变位于直肠、重度纤维化是导致ESD治疗早期结直肠癌、癌前病变术后迟发性出血的独立危险因素(P<0.05)。结论 对于直肠病变及黏膜下层重度纤维化的早期结直肠癌及癌前病变患者而言,行ESD治疗后易出现术后迟发性出血,临床需做好相关预防工作,以降低迟发性出血几率。Objective To explore the influencing factors of delayed bleeding after early colorectal cancer and precancerous lesions followed by endoscopic submucosal dissection(ESD).Methods 100 patients with early colorectal cancer and precancerous lesions were selected, and all patients were treated with ESD.The occurrence of delayed bleeding was observed and the factors associated with the occurrence of delayed bleeding after ESD treatment were analyzed.Results According to whether delayed bleeding occurred after ESD treatment, they were divided into bleeding group(11 cases) and no bleeding group(89 cases);the incidence of rectal lesions and severe fibrosis was higher than no bleeding group, and statistically different(P<0.05);Logistic regression analysis showed that the lesions were located in the rectum and severe fibrosis were independent risk factors for delayed colorectal bleeding after ESD treatment(P<0.05).Conclusion For patients with early colorectal cancer and precancerous lesions with rectal lesions and severe submucosal fibrosis, postoperative delayed bleeding is easy to occur after ESD treatment, and relevant prevention work should be done clinically to reduce the chance of delayed bleeding.
