高龄胃癌患者微创胃癌根治术后近期并发症发生的相关影响因素

摘　　要：目的 分析高龄胃癌患者行微创胃癌根治术后近期并发症发生的关联因素。方法 选取接受微创胃癌根治术的126例高龄胃癌患者,记录其术后近期并发症发生率。收集所有患者的年龄、性别、体重指数(BMI)、肿瘤直径、肿瘤部位等一般资料,运用多因素Logistic回归分析模型分析高龄胃癌患者行微创胃癌根治术后近期并发症发生的因素。结果 126例行微创胃癌根治术的高龄胃癌患者,术后1个月内,共有35例患者出现并发症,并发症发生率为27.78%(35/126)。单因素分析显示:性别、BMI、肿瘤直径、肿瘤部位、病理类型、浸润深度与高龄胃癌患者行微创胃癌根治术后近期并发症的发生无关(P>0.05);年龄、术前合并症、术中出血量、脉管侵犯与高龄胃癌患者行微创胃癌根治术后近期并发症发生有关(P<0.05);多因素Logistic回归分析显示:年龄≥80岁、术前有合并症、术中出血量≥200 ml、有脉管侵犯为近期并发症发生的主要关联因素(P<0.05)。结论 高龄胃癌患者行微创胃癌根治术后近期并发症发生率较高,且关联因素较多,如年龄≥80岁、术前有合并症、术中出血量≥200 ml、有脉管侵犯等。因此,临床需对上述关联因素予以高度重视,术前充分进行评估,尽可能降低并发症发生率。Objective To analyze the related factors of recent complications of minimally invasive radical gastrectomy in elderly patients with gastric cancer.Methods 126 elderly patients with gastric cancer who underwent minimally invasive radical gastrectomy were selected, and the incidence of recent postoperative complications was recorded;The age, sex, body mass index(BMI),tumor diameter, tumor location and other general data of all patients were collected.The factors of recent complications in elderly patients with gastric cancer after minimally invasive radical gastrectomy were analyzed by multivariate logistic regression analysis model.Results Among 126 elderly patients with gastric cancer who underwent minimally invasive radical gastrectomy, 35 patients developed complications within one month after operation, and the complication rate was 27.78%(35/126);Univariate analysis showed that gender, BMI,tumor diameter, tumor location, pathological type, depth of invasion were not related to the occurrence of recent complications after minimally invasive radical gastrectomy in elderly patients with gastric cancer, and the difference was not statistically significant(P>0.05);Age, preoperative complications, intraoperative bleeding, vascular invasion were related to the recent complications of minimally invasive radical gastrectomy in elderly patients with gastric cancer, and the difference was statistically significant(P<0.05);Multivariate logistic regression analysis showed that: age ≥ 80 years old(wald: 29.369,or: 16.111,95%ci: 5.896-44.025),preoperative complications(wald: 23.771,or: 10.560,95%ci: 4.094-27.238),intraoperative bleeding ≥ 200ml(wald: 22.105,or: 12.200,95%ci: 4.300-34.612) Vascular invasion(wald: 23.680,or: 16.569,95% ci: 5.348-51.333) was the main associated factor of recent complications(P<0.05).Conclusion The incidence of recent complications in elderly patients with gastric cancer after minimally invasive radical gastrectomy is high, and there are many related factors, such as age ≥ 80 years old, p
