血清白细胞介素-6、降钙素原对重症胰腺炎患者连续血液净化治疗预后的预测价值

摘　　要：目的探讨血清白细胞介素-6(IL-6)、降钙素原(PCT)对重症胰腺炎(SP)患者连续血液净化(CBP)治疗预后的预测价值。方法选取2018年1月至2021年1月于商丘市中心医院完成CBP治疗的96例SP患者为研究对象,患者均获得28 d随访结果,记录随访期间患者病死情况并作为预后观察指标;询问并记录一般资料及入院时血清IL-6、PCT水平及其他实验室指标[白细胞计数(WBC)、血小板(PLT)];绘制受试者工作特征(ROC)曲线分析血清IL-6、PCT水平对患者预后的预测价值。结果96例SP患者中,20例全因死亡,病死率为20.83%;病死组入院时急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)评分高于生存组(P<0.05);治疗前病死组血清IL-6、PCT水平均高于生存组(P<0.05);ROC曲线显示血清IL-6、PCT水平单独及联合预测SP患者CBP预后的曲线下面积(AUC)分别为0.833、0.764、0.884,具有一定的预测价值。结论血清IL-6、PCT水平预测SP患者CBP预后具有一定的价值。Objective To investigate the predictive value of serum interleukin-6(IL-6)and procalcitonin(PCT)on prognosis of patients with severe pancreatitis(SP)treated with continuous blood purification(CBP).Methods A total of 96 SP patients who completed CBP treatment in Shangqiu Central Hospital from January 2018 to January 2021 were selected as the study subjects.The results of 28 days follow-up of all patients were gained,the mortality of patients during follow-up was recorded and used as a prognostic index,asked and recorded baseline data,serum IL-6,PCT levels and other laboratory indicators[white blood cell count(WBC),platelet(PLT)]at admission.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of serum IL-6 and PCT levels on the prognosis of patients.Results Among 96 patients with SP,twenty cases died of all causes,and the fatality rate was 20.83%.The acute physiology and chronic health evaluation-Ⅱ(APACHEⅡ)score at admission in the death group was higher than that in the survival group(P<0.05).The levels of serum IL-6 and PCT in the death group before treatment were higher than those in the survival group(P<0.05).The ROC curve showed that the area under the curve(AUC)of serum IL-6 and PCT levels alone and in combination to predict the prognosis of SP patients with CBP were 0.833,0.764 and 0.884,respectively,which had certain predictive value.Conclusion Serum IL-6 and PCT levels have certain value in predicting the prognosis of patients with SP treated with CBP.
