鼻内窥镜下鼻腔泪囊吻合术治疗不同程度急性泪囊炎的临床效果

摘　　要：目的探讨鼻内窥镜下鼻腔泪囊吻合术治疗不同程度急性泪囊炎的临床效果。方法选取2016年7月至2020年12月我院收治的50例(50眼)急性泪囊炎患者作为研究对象,按照疾病程度不同将其分为轻度组32例(32眼)和重度组18例(18眼)。两组均给予鼻内窥镜下行鼻腔泪囊吻合术治疗。比较两组的治疗效果、并发症发生情况、泪膜破裂时间(BUT)、泪液分泌试验(SⅠT)、泪溢程度评分及美观度。结果两组的治疗总有效率比较,差异无统计学意义(P>0.05)。两组的鼻内渗血、鼻部滴血、感染发生率比较,差异无统计学意义(P>0.05)。术前,重度组的BUT、SⅠT长于轻度组,泪溢程度评分高于轻度组,差异具有统计学意义(P<0.05);术后6个月,两组的BUT、SⅠT均缩短,泪溢程度评分均降低,差异具有统计学意义(P<0.05);两组的BUT、SIT及泪溢程度评分比较,差异无统计学意义(P>0.05)。轻度组的美观率高于重度组,差异具有统计学意义(P<0.05)。结论鼻内窥镜下行鼻腔泪囊吻合术对不同程度急性泪囊炎均有较好的疗效,且并发症发生率低,术后美观率高,但重度患者可能遗留瘢痕,因此应及早行手术治疗。Objective To explore the clinical effect of nasal endoscopic dacryocystorhinostomy in the treatment of acute dacryocystitis of different degrees.Methods A total of 50 patients(50 eyes)with acute dacryocystitis admitted in our hospital from July 2016 to December 2020 were selected as the research objects.According to the degree of disease,the patients were divided into mild group with 32 cases(32 eyes)and severe group with 18 cases(18 eyes).Both groups were treated with nasal endoscopic dacryocystorhinostomy.The treatment effect,occurrence of complications,breakup time of tear film(BUT),schirmerⅠtest(SⅠT),tear overflow degree score and beauty of the two groups were compared.Results There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).There were no significant differences in the incidence rate of nasal bleeding,nasal drip and infection between the two groups(P>0.05).Before operation,the BUT and SⅠT in the severe group were longer than those in the mild group,and the tear overflow degree score was higher than that in the mild group,and the differences were statistically significant(P<0.05);six months after operation,the BUT and SⅠT in the two groups shortened,the tear overflow degree score decreased,and the differences were statistically significant(P<0.05);there were no significant differences in the BUT,SⅠT and tear overflow degree score between the two groups(P>0.05).The beauty rate in the mild group was higher than that in the severe group,and the difference was statistically significant(P<0.05).Conclusion Nasal endoscopic dacryocystorhinostomy has a good effect on acute dacryocystitis of different degrees,with low incidence of complications and high beauty rate,but severe patients may leave scars,so early surgery should be performed.
