摘　　要：目的:探讨视频眼震电图对脑小血管病伴头晕患者的前庭功能受损情况的评估方法和临床意义。方法:选取2020年1月—2022年1月南京医科大学附属苏州医院收治的年龄≥50周岁的21例脑小血管病伴头晕患者为CSVD组,选取同期CT或MRI头颅检查正常的健康体检及健康志愿者21例为对照组。两组均接受视频眼震电图检查,比较两组自发性眼震,凝视眼震,扫视,平滑跟踪及固视抑制异常的发生率。结果:C S V D组的自发性眼震,凝视眼震,扫视,平滑跟踪及固视抑制异常的发生率明显高于对照组,差异有统计学意义(P<0.05)。结论:CSVD组存在前庭中枢功能受损。视频眼震电图检查有助于进一步了解CSVD患者头晕的发病机制,对临床诊疗有一定的指导意义。Objective To investigate the evaluation method and clinical significance of vestibular function impairment in patients with cerebral small vessel disease.Methods From January 2020 to January 2022,21 patients aged≥50 years old with cerebral small vessel disease with dizziness admitted to the Affiliated Suzhou Hospital of Nanjing Medical University were selected as the CSVD group,and 21 healthy physical examinations and healthy volunteers with normal CT or MRI head examination during the same period were selected as the healthy control group.The two groups were underwent a video nystagmus.The incidence of spontaneous nystagmus,gaze nystagmus,saccade,smooth tracking and fixed vision inhibition was compared between the two groups.Results The incidence of spontaneous nystagmus,gaze nystagmus,saccade,smooth tracking and fixation suppression abnormalities was significantly higher in the CSVD group than in the control group,with statistically significant differences(P<0.05).Conclusion Patients in the CSVD group have impaired vestibular center function,and CSVD is an independent factor in the occurrence of dizziness symptoms in the elderly.Videonystagmography has a higher positive rate in patients with CSVD with dizziness,which is helpful to further understand the pathogenesis of dizziness in CSVD patients and has certain guiding significance for clinical diagnosis and treatment.
