摘 要:目的探索病理免疫组化方法检测MYCN蛋白的表达与荧光原位杂交法方法检测MYCN基因的一致性,初步建立价格低廉、操作简单且易于推广的MYCN检测手段,用于神经母细胞瘤患儿早期的分层诊疗。方法应用FISH及IHC共同检测NB患儿的肿瘤组织及转移骨髓的MYCN表达。结果共54例NB患儿纳入本研究,全部患儿的瘤组织均经FISH方法检测,其中MYCN基因扩增31例,非扩增23例;入组患儿中有25例患儿病初伴有骨髓转移,对转移骨髓应用FISH及IHC方法检测,17例患儿MYCN蛋白表达阳性,8例为阴性,与患儿瘤组织FISH方法检测结果完全一致;用IHC方法检测肿瘤组织及转移骨髓的MYCN蛋白,阳性一致率均为86%~94%,阴性一致率为100%。结论IHC方法对NB患儿的肿瘤组织及受累转移的骨髓进行MYCN蛋白的检测,其结果与FISH方法的检测结果具有较高的一致性,价格低廉、操作简单,适用于NB患儿早期的危险度评估及推广。Objective To explore the consistency of pathological immunohistochemical method to detect MYCN protein expression and fluorescence in situ hybridization method to detect MYCN gene,and initially establish a cheap,simple and easy to popularize MYCN detection method for early stratified diagnosis and treatment of children with neuroblastoma.Methods FISH and IHC were used to detect MYCN expression in tumor tissues and bone marrow metastasis in NB children.Results A total of 54 NB children were included in this study.Tumor tissues of all children were detected by FISH method.MYCN gene was amplified in 31 cases and non-amplified in 23 cases.Among the enrolled children,25 had bone marrow metastasis at the beginning of the disease.FISH and IHC methods were used to detect the bone marrow metastasis.MYCN protein expression was positive in 17 children and negative in 8 children,which was completely consistent with the results of FISH method in tumor tissues.IHC method was used to detect MYCN protein in tumor tissue and metastatic bone marrow.The positive and negative rates were 86%~94%and 100%,respectively.Conclusions The IHC method was used to detect MYCN protein in tumor tissues and bone marrow involved in metastasis of NB children.The results showed high consistency with those of FISH method,low cost and simple operation,which is suitable for early risk assessment and popularization of NB children.
