摘　　要：目的了解儿科肿瘤医生实施疼痛管理的现状,探索影响儿科肿瘤医生疼痛管理开展的障碍,为完善儿科疼痛管理提供循证依据。方法采用个人深度访谈法访谈16名儿科肿瘤医生,访谈结束后及时将录音转为书面文字,采用内容分析法进行资料分析。结果(1)儿科肿瘤医生对疼痛关注度普遍偏低,疼痛管理过程缺乏规范性。(2)儿科肿瘤医生实施疼痛管理障碍重重,包括医护人员、患儿及家长、药物获取与使用、文化方面、政策与管理等各方面。(3)改善肿瘤儿童疼痛管理现状的对策有:进行疼痛管理的宣传和培训;引入疼痛管理专业团队;国家政策支持。结论(1)医生及家长疼痛管理相关知识不足是疼痛管理的重大障碍,针对性培训非常必要。(2)目前缺乏适用于我国临床环境的疼痛管理模式或流程是实施疼痛管理的重要阻碍。Objective To understand the current situation of pain management by pediatric oncologists,find out the obstacles that affect the development of pain management by pediatric oncologists,so as to provide evidence-based database for improving pediatric pain management.Methods The interviewed doctors were selected through purposeful sampling,and 16 pediatric oncologists were interviewed using the personal in-depth interview method.After the interview,the recording was converted into written text in time,and conventional content analysis was applied to interview transcripts.Results(1)Pediatric oncologists generally pay less attention to pain,and the pain management process lacks standardization.(2)Pediatric oncologists have many obstacles in implementing pain management.The obstacles in children′s pain management are divided into medical staff,children and parents,drug acquisition and use,culture,policy and management.(3)Countermeasures to improve the current situation of pain management in children with cancer include:do related publicity and training on pain management;introduce professional teams of pain management;national policy support.Conclusions(1)Inadequate knowledge of pain management among physicians and parents is a major barrier to pain management,and targeted training is essential.(2)Currently,the lack of a pain management model or process suitable for the Chinese clinical environment is an important obstacle to the implementation of pain management.
