雷火灸配合穴位注射维生素B_(12)治疗脑卒中后肩手综合征的临床效果观察

摘　　要：目的:观察雷火灸配合穴位注射维生素B_(12)治疗脑卒中后肩手综合征(SHS)的临床效果。方法:选取南昌市洪都中医院2019年6月-2021年3月收治的60例脑卒中后SHS患者,依据随机数字表法分为对照组、试验组,各30例。对照组采用常规康复治疗,试验组在对照组基础上采用雷火灸配合穴位注射维生素B_(12)治疗,均治疗4周。比较两组疼痛程度[视觉模拟评分法(VAS)]、上肢功能[Fugl-Meyer运动功能评估量表上肢部分(FMA-U)]、肩关节活动度及日常生活能力[改良Barthel指数(MBI)]。结果:两组治疗后VAS评分均低于治疗前,且试验组低于对照组,差异均有统计学意义(P<0.05);两组治疗后FMA-U评分、肩关节前屈与外展度、MBI评分均高于治疗前,且试验组均高于对照组,差异均有统计学意义(P<0.05)。结论:脑卒中后SHS患者采用雷火灸配合穴位注射维生素B_(12)治疗可减轻疼痛程度,提高上肢功能、肩关节活动度及日常生活能力。Objective:To observe the clinical effect of thunder-fire moxibustion combined with acupoint injection of Vitamin B_(12)in the treatment of post-stroke shoulder hand syndrome(SHS).Method:A total of 60patients with post-stroke SHS who were admitted to Nanchang Hongdu Hospital of Traditional Chinese Medicine from June 2019 to March 2021 were selected and divided into a control group and an experimental group according to the random number table method,30 cases in each group.The control group was treated with conventional rehabilitation therapy,and the experimental group was treated with thunder-fire moxibustion combined with acupoint injection of Vitamin B_(12)on the basis of the control group.All the patients were treated for 4 weeks.The pain severity[visual analog scale(VAS)],upper limb function[Fugl-Meyer assessment scale upper limb part(FMA-U)],shoulder joint range of motion and daily living ability[modified Barthel index(MBI)]were compared between the two groups.Result:The VAS scores of the two groups after treatment were lower than those before treatment,and the experimental group was lower than that the control group,the difference were statistically significant(P<0.05).The FMA-U scores,shoulder joint range of anteflexion and abduction,and MBI scores in the two groups after treatment were higher than those before treatment,and the experimental group were higher than those the control group,the difference were statistically significant(P<0.05).Conclusion:Thunder-fire moxibustion combined with acupoint injection of Vitamin B_(12)in patients with post-stroke SHS can reduce the pain severity,improve upper limb function,shoulder joint range of motion and daily living ability.
