摘  要:目的:观察乳腺超声检查联合肿瘤标志物[癌胚抗原(CEA)、细胞角蛋白19(CK19)]检测在乳腺癌腋窝淋巴结转移中的临床价值。方法:回顾性分析2019年1月-2021年12月泰安市肿瘤防治院收治的100例原发性乳腺癌患者的临床资料。根据病理诊断结果,将患者分为转移组(n=59)与未转移组(n=41)。比较两组超声、肿瘤标志物检查结果;分析乳腺超声检查、肿瘤标志物检测结果诊断腋窝淋巴结转移的效能。结果:转移组的淋巴结长径≥7 mm、边界不清晰、内部回声不均匀、血流信号丰富、纵横径之比<2的占比均高于未转移组,差异均有统计学意义(P<0.05)。肿瘤标志物检测显示,转移组CEA、CK19水平均高于未转移组,差异均统计学意义(P<0.05)。联合检测敏感度高于超声检查,特异度低于超声检查,差异均有统计学意义(P<0.05)。联合检测的敏感度均高于CK19检测与CEA检测,差异均无统计学意义(P>0.05)。联合检测特异度高于CK19检测,但低于CEA检测,差异均无统计学意义(P>0.05)。结论:超声与肿瘤标志物联合检测可提高乳腺癌患者腋窝淋巴结转移诊断的敏感度,可建立以乳腺超声排查为基础,多检测项目联合应用的诊断流程,确保诊断结果准确。Objective:To observe the clinical value of breast ultrasonography combined with tumor markers[carcinoembryonic antigen(CEA),cytokeratin 19(CK19)]in axillary lymph node metastasis of breast cancer.Method:The clinical data of 100 patients with breast cancer treated in Tai’an Cancer Prevention and Treatment Hospital from January 2019 to December 2021 were retrospectively analyzed.According to the pathological diagnosis results,patients were divided into the metastatic group(n=59)and the non-metastatic group(n=41).The results of ultrasonography and tumor markers were compared between the two groups;the efficacy of breast ultrasonography and tumor marker detection in the diagnosis of axillary lymph node metastasis were analyzed.Result:In the metastatic group,the lymph node diameter≥7 mm,the boundary was unclear,the internal echo was uneven,the blood flow signal was rich,and the ratio of longitudinal and transverse diameter<2 was higher than those in the nonmetastatic group,the differences were statistically significant(P<0.05).Tumor marker detection showed that CEA and CK19 levels in the metastasis group were higher than those in the non-metastatic group,the differences were statistically significant(P<0.05).The sensitivity of combined detection was higher than that of ultrasonography,but the specificity was lower,the differences were statistically significant(P<0.05).The sensitivity of combined detection was higher than that of CK19 detection and CEA detection,the differences were not statistically significant(P>0.05).The specificity of combined detection was higher than that of CK19 detection,but lower than that of CEA detection,the differences were not statistically significant(P>0.05).Conclusion:The combined detection of ultrasonography and tumor markers can improve the sensitivity of diagnosis of axillary lymph node metastasis in breast cancer patients,and a diagnostic process based on breast ultrasound screening and the combined application of multiple detection items can be established to ensure accurate di
