摘  要:目的探讨^(18)F-FDG全身PET/CT动态显像评价MDA-MB-231乳腺癌裸鼠模型早期放疗效果的价值。方法建立MDA-MB-231乳腺癌裸鼠模型,按随机数字表法分为对照组和放疗组(每组10只),分别于放疗前和放疗完成后对裸鼠行全身^(18)F-FDG PET/CT动态显像,比较2组肿瘤SUV_(max)、最大示踪剂净流入速率常数(Ki_(max))的变化,计算靶本比(TBR),并记录肿瘤体积变化情况。以病理结果为参照,评估^(18)F-FDG全身PET/CT动态显像在评价裸鼠MDA-MB-231乳腺癌放疗效果中的价值。数据分析采用配对t检验、两独立样本t检验和Pearson相关分析。结果放疗后,放疗组SUV_(max)和Ki_(max)分别为4.66±0.46和0.14±0.03,均较放疗前降低(5.30±0.52和0.19±0.03;t值:4.61、8.31,P值:0.001、<0.001),对照组SUV_(max)和Ki_(max)(5.94±0.74、0.23±0.03)则较放疗前增加(5.24±0.50、0.19±0.02;t值:4.77、6.87,P值:0.001、<0.001)。2组所有扫描图像中TBR_(Ki)明显大于TBR_(suv)(14.11±5.58和5.91±1.60;t=8.92,P<0.001)。放疗组肿瘤体积较放疗前减小,但差异无统计学意义[(0.74±0.12)与(0.81±0.08)cm^(3);t=2.24,P=0.052]。免疫组织化学检测示,葡萄糖转运蛋白(Glut)1在肿瘤中高表达,放疗后放疗组Glut1阳性细胞百分率明显低于对照组[(38.30±6.18)%和(69.78±5.37)%;t=12.17,P<0.001]。Glut1表达与SUV_(max)、Ki_(max)均呈正相关(对照组:r_(suv)=0.75,P=0.012;r_(Ki)=0.77,P=0.010;放疗组:r_(suv)=0.67,P=0.035;r_(Ki)=0.77,P=0.010)。放疗组肿瘤细胞凋亡指数(AI)明显高于对照组[(24.15±4.00)%和(10.15±3.05)%;t=8.85,P<0.001]。结论^(18)F-FDG全身PET/CT动态显像可以敏感地监测裸鼠MDA-MB-231乳腺癌早期放疗效果。Objective To investigate the value of^(18)F-FDG total-body PET/CT dynamic imaging in evaluating the efficacy of early radiotherapy for MDA-MB-231 breast cancer bearing nude mice.Methods MDA-MB-231 breast cancer bearing nude mice were established and divided into control group and radiotherapy group based on the random number table method(n=10 for each group).^(18)F-FDG total-body PET/CT dynamic imaging was performed before and after the radiotherapy.The SUV_(max)and the maximum tracer uptake net influx constant(Ki_(max))of tumors,and the target-to-background ratio(TBR)were calculated.The change of tumor volume was recorded.The value of^(18)F-FDG total-body PET/CT dynamic imaging in evaluating the efficacy of radiotherapy was accessed using pathological findings as the reference.Paired t test,independent-sample t test and Pearson correlation analysis were used for data analysis.Results After radiotherapy,SUV_(max)and Ki_(max)(4.66±0.46 and 0.14±0.03)were reduced in the radiotherapy group compared with those before radiotherapy(5.30±0.52 and 0.19±0.03;t values:4.61,8.31,P values:0.001,<0.001),while the SUV_(max)(5.94±0.74 vs 5.24±0.50)and Ki_(max)(0.23±0.03 vs 0.19±0.02)were increased compared with baseline in the control group(t values:4.77,6.87,P values:0.001,<0.001).TBR_(Ki)was significantly higher than TBR_(suv)based on all images of the 2 groups(14.11±5.58 vs 5.91±1.60;t=8.92,P<0.001).The tumor volume in the radiotherapy group decreased compared with that before radiotherapy,but the difference was not statistically significant((0.74±0.12)vs(0.81±0.08)cm^(3);t=2.24,P=0.052).The results of immunohistochemistry showed that glucose transport protein(Glut)1 was highly expressed in tumors,and the Glut1 positive cell percentage of the radiotherapy group was significantly lower than that of the control group((38.30±6.18)%vs(69.78±5.37)%;t=12.17,P<0.001).The expression of Glut1 was significantly positively correlated with SUV_(max)and Ki_(max)(the control group:r_(suv)=0.75,P=0.012;r_(Ki)=0.77,P=0.010;the
