摘　　要：目的探讨外耳道腺样囊性癌的临床特点、诊断和治疗方法。方法回顾性分析2008年1月至2020年12月在扬州大学附属苏北人民医院收治的6例外耳道腺样囊性癌患者临床资料,分析其临床症状、治疗及随访结果。2例行乳突扩大切除术,3例行颞骨次全切+腮腺浅叶切除+颞肌修复术,1例行颞骨次全切+腮腺浅叶切除、深叶部分去除术+颞肌修复术。结果6例患者术后随访2~8年,随访5年以上者5例,术后复发1例。结论外耳道腺样囊性癌不易及时确诊,肿瘤生长缓慢,容易复发和发生肺部转移。手术治疗结合术后辅以放疗可延长患者生存期、降低复发,是治疗外耳道腺样囊性癌的有效方法。Objeetive To investigate the clinical characteristics,diagnosis and treatment of adenoid cystice carcinoma in external auditory canal.Methods A retrospective analysis was performed on 6 patients with adenoid cystic carcinoma in external auditory canal treated in Subei People's Hospital of Jiangsu Province from January 2008 to Dcember 2020.The clinical manifestation,treatment and follow-up results were analyzed.Treatments included mastoidectomy(n=2),subtotal temporal bone resection and superficial parotid gland resection and temporal muscle repain(n-3),sublotal temporal bone resection and superficial parotid gland resection and purtial deep parotid gland resection and temporal muscle repair(n-1).Results 6 patients were fllowed up for 2-8 years,4 patients were fllowed up for more than 5 years.One case had recurrence and one case had distant metastasis.Conclusions Adenoid cystic carcinoma of the external auditory canal is not easy to be diagnosed in a timely manner,the tumor growth is slow,it is easy to recur and prone to lung metastasis.Extended resection of the tumor with surgery plus postoperative adjuvant radiation prolong survival and reduce recurence.I's an efective treatment for adenoid cystice carcinoma of the extemal auditory canal.
