芎芷石膏汤加减联合盐酸氟桂利嗪治疗偏头痛疗效观察

摘　　要：目的 探讨芎芷石膏汤加减联合盐酸氟桂利嗪治疗偏头痛的疗效观察。方法 将2020年1月—2021年4月在医院神经内科治疗的108例偏头痛患者随机分为两组,对照组使用盐酸氟桂利嗪治疗,观察组使用芎芷石膏汤加减联合盐酸氟桂利嗪治疗,对比两组的临床疗效、偏头痛各指标、大脑血流速度、血清因子水平变化。结果 观察组治疗有效率为94.44%(51/54),明显高于对照组的74.07%(41/54)(P<0.05);观察组治疗后视觉模拟(VAS)疼痛评分、头痛频率、每次头痛持续时间均明显低于对照组(P<0.05);观察组治疗后大脑前动脉、大脑中动脉、大脑后动脉、椎-基底动脉血流速度明显低于对照组(P<0.05);观察组治疗后血浆内皮素-1(ET-1)、一氧化氮(NO)、神经肽(NPY)水平明显低于对照组,降钙素基因相关肽(CGRP)、脑脊液β-内啡肽(β-EP)水平明显高于对照组(P<0.05)。结论 芎芷石膏汤加减联合盐酸氟桂利嗪治疗偏头痛的疗效显著,能有效抑制偏头痛,减轻疼痛程度,减少发作次数,减慢脑血流速度,改善血管内皮功能,下调ET-1、NO、NPY水平,上调CGRP、β-EP水平,达到治疗偏头痛的作用。Objective To investigate the curative effect of Modified Xiongzhi Shigao Decoction(芎芷石膏汤加减)combined with flunarizine hydrochloride in the treatment of migraine.Methods A total of 108 migraine patients treated in the Department of Neurology of the hospital from January 2020 to April 2021 were randomly divided into two groups.The control group was treated with flunarizine hydrochloride,and the observation group was treated with Modified Xiongzhi Shigao Decoction combined with flunarizine hydrochloride.The clinical efficacy,migraine indexes,cerebral blood flow velocity and serum factor levels of the two groups were compared.Results The effective rate of the observation group was 94.44%(51/54),which was significantly higher than 74.07%(41/54)of the control group(P<0.05).Visual analogue scales(VAS)score,headache frequency and duration of each headache in the observation group were significantly lower than those in the control group(P<0.05).The blood flow velocities of anterior cerebral artery,middle cerebral artery,posterior cerebral artery and vertebrobasilar artery in the observation group were significantly lower than those in the control group(P<0.05).The levels of plasma endothelin-1(ET-1),nitric oxide(NO)and neuropeptide(NPY)in the observation group were significantly lower than those in the control group.The levels of cerebrospinal fluidβ-Endorphin(β-EP)and calcitonin gene related peptide(CGRP)were significantly higher than those in the control group(P<0.05).Conclusion Modified Xiongzhi Shigao Decoction combined with flunarizine hydrochloride is effective in the treatment of migraine.It can effectively inhibit migraine,reduce pain and the number of attacks,slow down the speed of cerebral blood flow,improve vascular endothelial function,down regulate the levels of ET-1,NO and NPY,and up regulate the levels of CGRP andβ-EP to achieve the effect of treating migraine.
