养血柔肝安神汤对帕金森病睡眠障碍的影响

摘　　要：目的 探讨养血柔肝安神汤治疗帕金森病睡眠障碍(PDSD)的疗效以及对中医症状、多导睡眠图(polysomnography, PSG)的影响。方法 选取2020年1月—2021年9月于六安市中医院就诊的80例PDSD患者,按入院顺序随机分为联合组与西药组,各40例。西药组接受常规西药治疗,联合组在西药治疗的基础上予养血柔肝安神汤治疗。连续治疗2周,比较两组疗效、中医症状、PSG参数及安全性指标差异,同时测定治疗前后同型半胱氨酸(Hcy)水平。结果 联合组总有效率92.50%(37/40),显著高于西药组72.50%(29/40),差异有统计学意义(P<0.05);治疗后,两组中医症状积分(入睡困难、烦躁易怒、头晕眼花、面色少华、神疲乏力、食欲低下)、PSG参数[睡眠潜伏期(SL)、觉醒的时间(AWT)、睡眠的效率(SE)、觉醒的次数(AT)、总的睡眠时间(TST)]及Hcy水平均明显改善,但联合组各指标改善情况优于西药组,差异有统计学意义(P<0.05);联合组、西药组两组不良反应发生率12.50%(5/40)、7.50%(3/40)比较差异无统计学意义(P>0.05)。结论 养血柔肝安神汤可提升PDSD疗效,缓解患者中医症状,调控Hcy水平,促进其睡眠结构改善,不失为治疗PDSD的有效手段。Objective To investigate the efficacy of Yangxue Rougan Anshen Decoction(养血柔肝安神汤)in the treatment of Parkinson's disease with sleep disorders(PDSD),and its influence on traditional Chinese medicine(TCM)symptoms and polysomnography(PSG).Methods Eighty patients with PDSD treated at Lu'an Hospital of Traditional Chinese Medicine between January 2020 and September 2021 were selected and randomly divided into combined group and western medicine group,with 40 cases in each group.Patients in the western medicine group were treated with conventional western medicine,while those in the combined group were treated with Yangxue Rougan Anshen Decoction on this basis.After 2 weeks of continuous treatment,the efficacy,TCM symptoms,PSG parameters and safety indicators were compared between the two groups.The level of homocysteine(Hcy)was measured before and after treatment.Results The total response rate of the combined group was 92.50%(37/40),significantly higher than 72.50%(29/40)of the western medicine group(P<0.05).After treatment,the scores of TCM symptoms(difficulty falling asleep,anxiety and irritability,dizziness,poor complexion,fatigue,poor appetite),PSG parameters[sleep latency(SL),awakening time(AWT),sleep efficiency(SE),awakening times(AT),total sleep time(TST)]and Hcy level were significantly improved,and the improvement in the combined group was better than that in the western medicine group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups[12.50%(5/40)vs 7.50%(3/40)](P>0.05).Conclusion Yangxue Rougan Anshen Decoction can improve the curative effect,relieve TCM symptoms,regulate Hcy level and promote the improvement of sleep structure in patients with PDSD.It is an effective treatment for PDSD.
