三种根充方法治疗窦道型根尖周炎超充情况分析

摘　　要：目的分析三种根管充填方法治疗窦道型根尖周炎患牙超充后的根尖区病变愈合情况。方法选取2018年3月至2019年12月杭州师范大学附属医院收治的150例窦道型根尖周炎患者纳入研究,根据随机数字表法将其分为三组,每组各50例。A组患者采用iRoot SP单尖法进行根充,B组患者采用iRoot SP热牙胶进行根充,C组患者采用AH-plus热牙胶进行根充。采用X线片观察三组患者的根管超充情况,比较三组根管超充患者术后1年的临床疗效。结果A、B、C组患者术后的超充发生率分别为56%、68%和48%,三组患者比较差异有统计学意义(χ^(2)=4.142,P=0.126);1年后,A、B、C组根管超充患者的根管治疗有效率分别为100%、73.53%和33.33%,三组比较差异有统计学意义(χ^(2)=28.040,P<0.001)。结论三种根管充填方法治疗窦道型根尖周炎,AH-plus热牙胶超充率最低,超充1年后根尖病变有效率iRoot SP单尖法最好,iRoot SP超充并不影响根尖病变愈合,反而可促进愈合,值得临床开展应用。Objective To analyze the healing of three root canal filling methods in the treatment of sinus periapical periodontitis after overfilling.Methods A total of 150 patients with sinus periapical periodontitis admitted to the Affiliated Hospital of Hangzhou Normal University from March 2018 to December 2019 were included in the study and divided into three groups according to random number table method,with 50 cases in each group.The patients in group A were filled with iRoot SP single tip method,the patients in group B were filled with iRoot SP hot gum,and the patients in group C were filled with AH-plus hot gum.X-ray films were used to observe the root canal overfilling of three groups,and the clinical efficacy of three groups of patients with root canal overfilling was compared one year after surgery.Results The incidence of postoperative overfilling in group A,B and C were 56%,68% and 48%,respectively.There was a significant difference among the three groups(χ^(2)=4.142,P=0.126).One year later,the effective rates of root canal therapy in groups A,B and C with root canal overfilling were 100%,73.53% and 33.33% respectively,with a statistically significant difference among the three groups(χ^(2)=28.040,P<0.001).Conclusion Three root canal filling methods for the treatment of sinus periapical periodontitis,AH-plus hot gutta percha has the lowest overfilling rate,and the iRoot SP single tip method has the best effect on apical lesions after overfilling for one year.iRoot SP overfilling does not affect the healing of apical lesions,but can promote the healing,which is worthy of clinical application.
