摘 要:目的探讨南京市乙肝病人乙型肝炎病毒(HBV)-P区突变特点与肝癌易感性的相关性。方法选取2018年1月至2020年12月南京市第二医院乙肝病人1100例,所有病人均采用聚合酶链反应(PCR)检测HBV基因分型及其P区突变位点情况,结合病理检查明确肝癌的发生情况,分析HBV-P区突变特点与肝癌易感性的相关性。结果结合病理检查结果,纳入的1100例的乙肝病人中,肝癌发生率为2.18%(24/1100);肝癌组HBV-P区突变率高于乙肝组,差异有统计学意义(P<0.05);进一步分析HBV-P区突变特点发现,肝癌组rt204、rt180、rt250位点突变率高于乙肝组,差异有统计学意义(P<0.05);logistic回归分析结果显示,rt204、rt180、rt250位点突变是乙肝病人发生肝癌易感性的独立影响因素(P<0.05)。结论南京市乙肝病人HBVP区突变特点与肝癌易感性有关,其中rt204、rt180、rt250位点突变可能易导致肝癌发生。Objective To investigate the correlation between hepatitis B virus(HBV)-P zone mutation and hepatocellular carcinoma(HCC)susceptibility in patients with hepatitis B in Nanjing.Methods A total of 1100 patients with Hepatitis B from January 2018 to December 2020 in Nanjing Second Hospital were selected.All patients were tested for HBV genotyping and mutation sites in P zone by Polymerase chain reaction(PCR).The occurrence of liver cancer was determined according to pathological examination results,and the correlation between HBV-P zone mutation characteristics and liver cancer susceptibility was analyzed.Results According to the results of pathological examination,the incidence of liver cancer was 2.18%(24/1100)in 1100 patients with hepatitis B.The mutation rate of HBV-P region in liver cancer group was higher than that in hepatitis B group,and the difference was statistically significant(P<0.05);Further analysis of the mutation characteristics of HBV-P region showed that the mutation rates of rt204,rt180 and rt250 sites in liver cancer group were higher than those in hepatitis B group,and the difference was statistically significant(P<0.05);Logistic regression analysis showed that rt204,rt180 and rt250 mutations were independent factors influencing the susceptibility of patients with Hepatitis B to HCC(P<0.05).Conclusion The mutation characteristics of HBV-P region in Nanjing patients are related to the susceptibility to liver cancer,among which the mutation of rt204,rt180 and rt250 sites may easily lead to the occurrence of liver cancer.
